Licensing Program Analysts (LPAs) Christopher Bello and Julia Maryanova arrived at the facility at approximately 10:30am met with Site Supervisor, Belva Gonzalez, regarding an Unusual Incident that took place 12/5/2024. LPAs made observations and interviewed staff regarding the incident.

Site Supervisor stated that the incident was an accident and the child most likely landed wrong while playing. The facility already had a meeting regarding safety and supervision while using the play structure.

No Title 22 Deficiencies observed in the areas that were evaluated. LPA reviewed report with the director and provided copies. An exist interview was conducted. LPA observed the Notice of Site Visit posted and the director understands it must remain posted for 30 days.
