13 | Licensing Program Analyst (LPA) Jennifer Velasco met with Facility Representative Liliana Lopez (Representative), for the purpose of conducting a subsequent complaint investigation inspection and to deliver findings. It was alleged that an authorized representative was denied access to the facility. During the investigation, LPA conducted interviews, reviewed documents, and observed staff provide care to children. According to staff interview, locking the gate was considered necessary to prevent unhoused adults from entering the facility. Staff witnesses stated now cones are used. Staff, parent, and child interviews supported the allegation that parents could not enter the facility without staff assistance. Documentation showed at least one parent was required to contact the facility prior to arriving for pick-up/drop-off. Based on witness statements and document review, the preponderance of evidence standard has been met, and the allegation is substantiated. A Title 22 deficiency is cited on the subsequent page of this report. Exit interview was conducted, this report was reviewed with Facility Representative, and appeal rights were provided. Facility Representative's signature on this report acknowledges receipt of these rights. Notice of site visit was provided and shall remain posted for a period of 30 days. |
