At 2:00 p.m. on Thursday, April 7th, 2022, Licensing Program Analysts (LPAs) Karyn Guerra and Amanda Sutter met with Licensee, Danelle Ingalls, for the purpose of an unannounced complaint inspection. It was alleged that the menu is not posted. Throughout the course of the investigation, LPA conducted an interview and made observations. A menu was not observed posted at the facility during inspection. Licensee stated that it is usually posted on the side of the cubby area, but they did not have one up for the current week. The menu is usually prepared weekly and is the same for the preschool program and school age option. The preponderance of evidence standard has been met, and the allegation is substantiated. Regulatory violation is cited on the subsequent pages of this report. Appeal rights were provided. Licensee's signature acknowledges receipt of these rights. An exit interview was conducted and a notice of site visit provided. Notice of site visit shall remain posted for 30 days.
