On Wednesday, May 14, 2024, at approximately 2:30 PM Licensing Program Analyst (LPA) Josiah Gathing met with Facility Representative Renee Perez, for the purpose of a complaint investigation and to deliver findings. It was alleged that staff do not provide adequate food service and staff do not conduct required fire drills. Throughout the course of the investigation, LPA conducted interviews, reviewed documents, and made observations.
Staff stated in interview that meals provided to the children often do not match the menu. Director stated that the menu is subject to change. LPA advised Director that the menu should be followed as often as possible and any changes should be documented.
Staff interviews disagreed whether fire drills were conducted within six months. Director was unable to produce a fire drill log upon request.
Therefore, based on record review, the preponderance of evidence standard has been met, and the allegation is substantiated. Deficiencies are cited on the subsequent page of this report.
An exit interview was conducted and a notice of site visit provided. Notice of site visit shall remain posted for 30 days. Appeal rights were provided to the Facility Representative.

On Wednesday, May 14, 2024, at approximately 2:30 PM Licensing Program Analyst (LPA) Josiah Gathing met with Facility Representative Renee Perez, for the purpose of a complaint investigation and to deliver findings. It was alleged that licensee is operating in violation of the facility fire clearance. Throughout the course of the investigation, LPA conducted interviews, reviewed documents, and made observations.
Staff stated in interview that the fire inspector instructed Director to remove curtains not treated for fire resistance. Fire inspector stated in interview that materials like curtains must be removed or treated for fire resistance. LPA determined in interview with fire inspector that the fire clearance for the facility was not revoked. Therefore, the facility is not operating in violation of fire clearance.
Although the alleged violations may have happened or are valid, the preponderance of evidence standard has not been met to fully prove or disprove that they did or did not occur, therefore, they are unsubstantiated. An exit interview was conducted and a notice of site visit provided. Notice of site visit shall remain posted for 30 days.
