At 2:30 p.m. on Tuesday, May 10th, 2022, Licensing Program Analyst (LPA) Karyn Guerra met with Director, Anna Royal, for the purpose of a complaint inspection and to deliver findings. It was alleged that unqualified staff are providing supervision to day care children. Throughout the course of the investigation, LPA conducted interviews and made observations. During today's inspection, LPA observed a census of 6 Toddler children supervised by 2 staff (S1, S2) in the toddler option of the program. It was learned during interviews that neither staff is a qualified teacher. It was also alleged that the facility is operating out of ratio. During today's inspection, LPA observed a census of 21 preschool children supervised by 2 staff (S3, S4). In a file review conducted, S3 was found to be a fully qualified teacher. In an interview conducted, S4 was stated to not have any units. It was alleged that staff are not allowing parents to visit daycare children. Throughout the course of the investigation, LPA conducted interviews and made observations. During

initial inspection to the facility, LPA observed a doorbell and locked front entry door to the facility. LPA observed staff escort children to and from the front door as parents arrived for pick up. LPA came to learn that the facility did this as a COVID-19 practice. It was stated that during that time, parents were not allowed into the facility. LPA came to learn via interviews that if parents were to ask to enter the facility, they would not be allowed to, as per the directive from the prior Director. The facility has since lifted the restriction and parents now utilize a code to enter the facility for drop off and pick up of children. The preponderance of evidence standard has been met, and the allegations are substantiated.

Title 22 deficiencies are cited on the subsequent pages of this report. Director acknowledges, that FOR TYPE A DEFICIENCIES ONLY upon receipt, licensee shall post LIC 9099D with Type A deficiencies for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the licensee. LIC 9224 and Appeal Rights were provided. Director's signature on this report acknowledges receipt of these rights. This report was reviewed with the Director, Anna Royal. An exit interview was conducted. A Notice of Site Visit was provided and shall remain posted for a period of 30 days.

The program has allowed parents back into the facility. LPA observed parents entering into facility during pick up time in the evening. It was also stated that an email was sent to parents to notify of the update. POC cleared during inspection.
