At 11:00 a.m. on Thursday, October 14th, 2021, Licensing Program Analyst (LPA) Karyn Guerra met with Director, Reonna Walker, for the purpose of a complaint inspection to deliver findings. It was alleged that teachers are not fully qualified. During the course of the investigation, LPA conducted interviews, made observations, and reviewed records. Through interviews and file review, LPA came to learn that the facility has some staff who are are not fully qualified. During interviews, it was stated that staff without units are not left alone with children other than during nap time or to escort a child to the restroom.The allegation is unsubstantiated. Although the alleged violation may have happened or is valid, there is not enough evidence to fully prove or disprove that the alleged violation occurred, therefore the allegation is unsubstantiated. An exit interview was conducted, and a notice of site visit and appeal rights provided. Notice of site visit shall remain posted for a period of 30 days for parental review.

At 11:00 a.m. on Thursday, October 14th, 2021, Licensing Program Analyst (LPA) Karyn Guerra met with Director, Reonna Walker, for the purpose of a complaint inspection to deliver findings. It was alleged that the facility commingles children. During the course of the investigation, LPA conducted interviews and made observations. The facility has a toddler component designated in room 4, named the Seedlings. LPA came to learn that toddler children from the Seedlings classroom combine with Sprouts classroom for children aged 2 and 1/2 to 3 years. LPA also learned that older preschool children from the Sequoia classroom aged 3 and 1/2 to 4 and 1/2 combine with the Sprouts and Seedling classrooms in the Sprouts classroom for naptime. Based on observations and interviews, the preponderence of evidence standard has been met, and the allegation is substantiated. A Title 22 deficiency is sited on the subsequent pages of this report. An exit interview was conducted. Appeal rights and a notice of site visit were provided. Notice of site visit shall be posted for a period of 30 days for parental review.

Director stated that Seedlings toddler children will nap in Seedlings classroom (room 4) and will not commingle with the Sprouts classroom. Director will request in writing for a commingling waiver for the first and last hour.

Based on observation and interviews, the Seedling toddler component was commingling with children from older classrooms. This poses a risk to the health and safety of children in care.
