25 | LPA arrived to the facility to open a complaint. During the inspection, LPA observed deficiencies that were unrelated to the complaint allegations. At 8:50am, LPA observed 7 toddlers in the toddler room supervised by one staff (S1). This teacher did not have infant units. The ratio in a toddler classroom is 6:1 and the room was observed to be out of ratio at the time of the inspection. LPA was told an additional teacher was due to arrive at 9am so the director stepped into the classroom to bring the room into ratio until the additional teacher arrived. The Redwood classroom was observed to be out of ratio with 24 children an aid and a fully qualified teacher (FQT). The outside playground was out of ratio with and aid and a FQT supervising 29 children. LPA observed staff (S2) was present in the facility today and did not have a fingerprint clearance. The employee has been working in the facility for more than 5 days, therefore Civil Penalties are assessed in the amount of $500.
Title 22 Deficiency has been cited on the attached LIC 809-D. LPA Amanda Blesi informed facility representative, Reonna Walker, that this report dated 01.05.22 document(s) two Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.
Also, LPA Amanda Blesi informed the facility representative to provide a copy of this licensing report dated 01.05.22 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Appeal Rights given.
Exit interview conducted and report was reviewed with the Licensee Hamid Hosseini who arrived towards the end of the inspection when the director had to leave. A notice of site visit was given and must remain posted for 30 days.Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
