14 | Based on interviews and documentation review, LPA determined that licensee failed to ensure the constant direct visual observation and supervision by staff which resulted in Infant #1 being bitten on the cheek when Teacher Staff #1 had their back to the infant while diaper changing another infant in care. This lack of supervision/visual observation poses an immediate health and safety risk to children in care. | 8

14 | Based on interviews, LPA determined that licensee failed to ensure that infant care aides only work under the direct supervision of the director, the assistant director or a fully qualified teacher, except as provided for in Section 101416.5(d)(1) pertaining to napping infants. During interviews, LPA discovered that on several occasions licensee failed to ensure that only staff qualified to care for infants were left alone to do so. This poses a health and safety risk to infants in care. | 8
