On Thursday, May 19, 2022, at approximately 1:15 PM, Licensing Program Analyst (LPA) Alize Tillery met with Floating Director, Hannah Verigin, to conduct an unannounced inspection to initiate the complaint investigation regarding the above allegations. During today's visit, LPA observed 30 preschool children supervised by 4 staff. Administrator Alix Hall arrived to the facility to assist at approximately 2:30 PM.

During today's inspection, LPA Tillery toured all activity areas: Magnolia, Coconut, Oaks and Redwood classrooms. One of the allegations is that card board boxes are placed in between napping children. Upon arrival, LPA observed this to be true, as napping children in room Oaks were napping with card board boxes hendering the teachers view of the children. Administrator stated that this was happening due to parent request of social distancing. This causes a lack of supervision and poses a potential risk in case of emergency exit was necessary.
The second allegation was that the facility does not store food properly. During LPA's investigation, the kitchen was toured and LPA observed molded and expired foods. This poses a potential risk to children. Floating Director stated that the cook has recently left the facility and that herself and Administrator are working on creating a plan to keep the kitchen current and operating sufficiently.

On Thursday, May 19, 2022, at approximately 1:15 PM, Licensing Program Analyst (LPA) Alize Tillery met with Floating Director, Hannah Verigin, to conduct an unannounced inspection to initiate the complaint investigation regarding the above allegations. During today's visit, LPA observed 30 preschool children supervised by 4 staff. Administrator Alix Hall arrived to the facility to assist at approximately 2:30 PM.

During today's inspection, LPA Tillery toured all activity areas: Magnolia, Coconut, Oaks and Redwood classrooms. It was alleged that there is unqualified staff providing care at the day care center. Based on LPA observations of the staff files, and conflicting information obtained during staff interviews, there is not a preponderance of evidence to prove that the alleged violation occurred. Although the allegation may have happened or is valid, the allegation is UNSUBSTANTIATED.

LPA reviewed the report with Director and appeal rights were provided.
A notice of site visit was given an must remain posted for 30 days.

(19) All food shall be protected against contamination. Contaminated food shall be discarded immediately.
This requirement was not met, evidenced by: LPA observed molded food in the fridge and expired dry foods in the pantry.

Administrator and Floating Director will go through all foods to ensure molded and expired foods are discarded. Administrator and Floating Director will come up with a schedule for the kitchen while they look to hire a new cook.
Administrator will send proof of kitchen schedule to LPA Tillery.
