On 02/02/2022 at 1:30 PM Licensing Program Analysts (LPAs) Jeevun Birk-Miller and Katy Maestas conducted an unannounced complaint investigation to deliver the findings for the above allegations. LPAs met with Director, Bonnie Yip. Throughout the course of the investigation LPAs conducted observations, interviews, reviewed and collected documents. It was alleged that the facility staff redistributed used napping sheets to children after the unwashed sheets were placed in the dryer. It was stated by the Director that the washer had broken and has since been fixed. The Director stated since that happened, they changed the wash schedule to once a week and each sheet is stored in each child’s cubie. The same sheet stays with the same child who used it. Four of four staff stated staff had been taking turns taking the sheets home to wash. One of four staff stated once someone had the idea to dry the sheets after use to clean the bacteria and then sheets were given to different children.

It was alleged that the facility did not serve day care children milk for lunch. LPAs observed milk being served during breakfast snack on 1/13/2022. Director stated they strictly provide milk and water for the children. Continue to 9099-C page.

She stated during special occasions juice is served. Four of four staff stated that milk is provided at the facility. Although the facility does provide milk on occasions LPAs observed that the portions served did not meet the minimum serving requirement. During interviews with staff and parents of children in care it was stated that the quantity of the food served was not adequate. It was stated when children asked for more food they were told no. LPAs did an observation of the kitchen and observed that the food being prepped did not meet the required portions for children in care. When served they did not receive the initial amount that is to be served. LPA gathered photos of food items.

It was alleged a child is spitting on other children and nothing is being done. It was alleged that the incidents were documented, but conversations were not had and nothing further had been done. Through file reviews of two children's files it was observed that incident reports had been written regarding behaviors, such as spitting on and hitting other children. Staff interviewed stated when behaviors in the classroom happened staff addressed it with the children, but there was not consistency in staff addressing the behavior. Through interview with parents when incidents occurred in the classroom there was no communication with staff or resolution to the incidents that were documented and behaviors continued without modification.

Based on interviews, file reviews, and observation conducted the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. The following Type B citations were given during the inspection on the LIC9099-D page of this report. An exit interview was conducted with the Licensee. A Notice of Site Visit was posted by LPA Birk-Miller and this shall be posted for 30 days.
