25 | At 12:05 p.m. on Thursday, February 20th, 2020, Licensing Program Analyst (LPA) Karyn Guerra met with Licensee, Hiresha De Silva, for the purpose of an unannounced annual random inspection. Census consisted of 6 infant children supervised by 2 staff, and 15 toddler children supervised by 4 staff. Facility hours of operation are 7:00 a.m. to 6:00 p.m., Monday thru Friday.
LPA toured the building including all activity and classroom spaces, restrooms, food service, and outdoor play areas. Currently, no children in care require medications. Licensee stated there are no poisons on the premises. Toxic and hazardous items are inaccessible to children. Furniture and equipment are in good condition. Playground equipment and surfaces are free of loose or sharp parts. LPA observed foam cushioning under play structure, and adequate shading provided by trees. LPA observed fencing in back portion of yard along the side of the building to be in disrepair. Licensee stated that the neighboring property is doing construction and has damaged the fence. Licensee stated that Church administration is in communication with neighboring property to fix the fence. Licensee stated that children are not allowed in the portion of the play yard with the damaged fencing and she is constructing new fencing to block off the area. LPA observed wooden fencing replacement that Licensee is preparing to be installed. Licensee requested to make side yard off limits. LPA provided form LIC999 Facility Sketch to Licensee to update facility sketch. Infant changing table is located within arm's reach of a sink and has a padded surface that is washable and at least one-inch-thick. The floors appeared clean throughout the facility. The food preparation space is free of litter, and all food was protected against contamination. Storage containers with solid waste have tight-fitting covers. Infant bottles and food in refrigerator were labeled with child's name. Licensee was reminded to include date on labeling. Menus were posted and drinking water was readily available to children both indoors and outdoors via water cooler and bottles. LPA observed full legal signatures while reviewing the sign in and sign out binder.
