On 03/03/2023, at 9:00 am Licensing Program Analyst (LPA) Corina Beckby met with Licensee, Loralee Luminare for an Unannounced Annual Inspection. Present during the inspection was the Licensee, her mother and father. There were 12 children present at the time of inspection. Licensee's operating hours are Monday through Friday from 7:00 AM. to 5:30 PM. Licensee made modifications to the backyard adding a studio shed as a classroom and a patio overhanging for the play area. Licensee would like to change the yard from off limits to on limits. LPA verified the annual fees are current.

Licensee stated there are no new residents in the home since licensure. All Adult residents have criminal record clearances. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.



A health and safety inspection was conducted in all areas accessible to children. LPA and Licensee toured the single-story 3 bedrooms, 2 bathroom home owned by the Licensees. Off-limit areas include the master bedroom, the children’s bedroom laundry room, and garage. Off limit areas are being made inaccessible by closed locked doors and baby gates. Licensee acknowledged that children may never enter these off-limit areas. LPA observed the required postings and a working phone, and a 2A10BC fire extinguisher located on the wall next to the front door, meeting regulations. There is a fire alarm on the wall next to the front door as well. LPA observed smoke and carbon monoxide detectors, and verified they were both functional. LPA toured the kitchen area and verified knives and cleaners were inaccessible to children in care. The fireplace in the living room has been blocked off with furniture and made inaccessible

to the children in care. The home is equipped with central heating and air for proper ventilation. Licensee has stated there are no more pets. Licensee stated there are no weapons in the home. There are no bodies of water on the premises.

The inside and outside of the home are observed to be neat and clean with ample age appropriate materials for the children. LPA observed cleaning supplies properly stored underneath the kitchen sink that is secured with a lock and out of reach of children. LPA observed medications are stored top cabinet in the kitchen and out of reach of children, LPA observed age-appropriate toys for children in the living room. Toys appear to be clean and safe. The backyard is fenced and a studio shed was added as an additional learning environment. This shed is pending approval from the fire marshall. The shed has a heater, air conditioning unit, working smoke alarm, carbon monoxide detector and a 2A10BC fire extinguisher. A patio overhang was also added on the side of the house as additional shade for the children. Licensee understands that prior to making alterations or additions to the home or grounds, the licensee shall notify the Department of the proposed changes.



Six Children's files were reviewed. Emergency information and required immunization records were on file. LPA observed a current roster and documentation that a fire drill is conducted at least once every six months. The last disaster drill was documented on 1/12/23. Licensee's immunization records for measles (MMR), pertussis (Tdap), and the flu are available in the facility file. Current in-person EMSA pediatric CPR and First Aid certification was verified and expires 1/22/2025 for Lorna Doniza and Child Care Provider Mandated Reporter certification was verified and expires 11/5/23 for Licensee. Licensee was reminded that EMSA approved Pediatric CPR & First Aid training must be completed every two (2) years. Children’s Roster must be properly maintained.

In addition, LPA discussed the infant sleep regulations with licensee. LPA discussed the requirement to check and log infant napping every 15 minutes for infants 24 months and under. LPA provided a copy of LIC 9227 Individual Sleeping Plan, for infants under 12 months, during today's inspection. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices

This provider is not currently providing Incidental Medical Services (IMS) services to children in care. IMS policy was discussed. Licensee was reminded that California Law requires licensees to report unusual incidents or injuries to children in care, to child's parents, and to the Department of Social Services using the Unusual Incident/Injury form (LIC 624). Incidents must be reported within 24 hours by phone, fax, or email. Licensee was encouraged to visit the Department website at WWW.CCLD.CA.GOV for childcare updates, current forms, legislation and regulation information.
During today's inspection LPA observed the facility home and yard sketches which were updated. As of today's date, 3/03/2023, studio shed will not be used pending fire marshal approval.

An exit interview was conducted, and report was reviewed with the licensee, Loralee Luminare. A copy of this report will remain on file for a period of three years for public review upon request. Licensee's signature on this form acknowledges receipt of this form. A notice of site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
In the areas that were evaluated, no deficiencies were cited during today’s inspection.
