13 | Licensing Program Analysts Erwina Pascual-Golamco and Jennifer Velasco (LPA) met with Owner Mercy Ishida to deliver findings. LPAs toured the facility, including all activity and classroom spaces, restrooms, food service and outdoor play areas. Census included 27 preschool children being supervised by 4 classroom staff. Owner was reminded never to exceed the conditions, limitations, and capacity specified on the license. Facility hours of operation are Monday through Friday from 7:00 AM to 6:00 PM.
It was alleged that a daycare child sustained an injury while in care due to lack of supervision, and that staff were not providing adequate supervision to daycare children. Throughout the course of the investigation, LPA observed staff provide care to children, conducted interviews, and reviewed documents. Staff interviews revealed that minor injuries are treated with first aid and reported to parents through the Building Kidz application. Parents stated in interviews that they receive updates on their children through the Building Kidz application as well as in person, or a phone call. Witness statements, LPA observation, and facility documentation failed to corroborate the allegation that a daycare child sustained an injury while in care due to lack of supervision, and that staff were not providing adequate supervision to daycare children. Although the alleged violations may have happened or are valid, the preponderance of evidence standard has not been met to fully prove or disprove that they did or did not occur, therefore, they are unsubstantiated.
An exit interview was conducted and a notice of site visit provided. Notice of site visit shall remain posted for 30 days. |
