25 | On 2/18/22 Licensing Program Analyst (LPA) Fabiola Diaz met with licensee Eriquetta Johnson for unannounced annual/1 year inspection. Present in the facility were 3 day care children and 1 child that lives in the home.
The two story home has an unfenced front yard, 4 bedrooms, 3 bathrooms, kitchen, family room, laundry room, garage, dining room, living room, and fenced backyard. Off-limits areas in the home are: upstairs, dining room, living room, laundry room, right & left sides of the backyard, and garage. Off-limits areas will remain inaccessible to children by closed doors and/or supervision. Licensee acknowledges that children may never enter these off-limit areas. Licensee was notified that prior to use of any off limits area, the department must be notified.
A health and safety inspection was conducted in the areas accessible to children. The house has a working telephone, fully charged fire extinguisher, smoke detector and carbon monoxide detector that meet regulations. Licensee stated there are no weapons in the home. LPA observed all the required postings. LPA advised the licensee that if there are any poisons at the home, all poisons must be locked with a key lock or combination lock. LPA observed no bodies of water on premises. LPA observed barricaded stairs. The fire place was also barricaded, and licensee stated that it is not used during day care hours.
Licensee's immunization records are in the home facility file. Children's roster and a fire drill log were observed. Licensee's CPR/First aid card had an expiration date of 2/11/22. Licensee explained she is enrolled to take the training on 2/28/22 to renew it. Mandated Reporter Training for licensee expires on 3/3/22. Licensee understands training must be complete every two years. LPA observed that children's files were missing the Affidavit LIC282. LPA discussed the LIC282 and the option to acquire liability day care insurance.
