Licensing Program Analyst (LPA) Gagandeep Singh and Licensing Program Manager (LPM) Seychelle De Luca conducted an announced pre licensing inspection with Applicant Amber Lashley, Director Latisha King and Liz Donkor. Due to COVID-19 precautions, this inspection was conducted virtually using ‘Facetime’ application on cell phone. This is a combination center of Infant program (343614528) and preschool age program (343614527). Applicant requests a school-age license to serve eight school-age children from kindergarten and above. The Department has received the approved fire clearance from local fire department. As the facility has been approved for a waiver to serve school age children at this time, there are six school age children present today.

INDOOR ACTIVITY SPACE:
During the inspection LPA and LPM inspected the facility for any health and safety hazards. The facility will be using one classroom for school age children. LPA observed a sufficient number of tables, chairs, and cubbies available for children. LPA observed a variety of age-appropriate toys and equipment. There is a first aid kit in available in the office. Medications will be stored in the office in a locked box. LPA observed that there is drinking water available for children in the classroom. There is fire extinguisher in the hallway. LPA measured the classroom. The total classroom space contains a total of 361.038 square feet, which will accommodate ten school-age children. There are three toilets, three urinals and five sinks for the children. Toilet have privacy and gender separations. LPM asked the applicant to cover the restroom door glass to provide complete privacy to the children. There is a separate private restroom for the staff in staff room. Individual measurements are recorded on the Capacity Worksheet (LIC 9024). Children who become ill during the day will be isolated in the office area. Per applicant, facility will provide the meal. LPA observed the meal menu being posted in the classroom.

OUTDOOR ACTIVITY SPACE:
The applicant informed that facility will be using same play yard for school age children and preschool age children, but at the different times. Applicant is informed that the facility is required to have waiver to share the play yard. The outdoor play area is fenced with a metal fence that appears to be at least four feet tall. LPA observed a variety of age-appropriate outdoor toys. There is no school age play structure in the play yard. LPA informed the applicant that if facility add a play structure for school children, it must inaccessible for preschool children. There is plenty of shaded area available for children. Play yard has water fountain available to provide the drinking water to the children. As there has been no change in the play yard from preschool program, the play yard was not measured today. Play yard was measured in 2007 as 9464.47 square feet of area.

Applicant is aware that the following documents must be posted at all times: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, Effects of Lead Exposure, car seat poster, menus, and daily schedule. LPA observed facility has COVID-19 precautions and guideline posters visible at the main entrance. LPA observed facility has thermometer, sanitizer, face masks and PPE supplies available at the front desk.Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following items are required before a license will be issued:
- Updated application to request the capacity of ten.
- Personal Report with original signature.
- Proof of installing Carbon Monoxide Detector.
- Cover the glass on bathroom door for privacy.
This facility evaluation report was reviewed and discussed with Applicant. Copy of the report was provided through an email and confirmation was received. Applicant was encouraged to the visit the Department's website at WWW.CDSS.CA.GOV for information regarding child care updates, forms, regulations and legislation pertaining to child care centers.
