Application Specialist (AS) Mai Lor and Licensing Program Analyst (LPA) Jan Hoshida met with Applicant Justina Onumajuru, Director Nicole Bunch, and Assistant Director Nyika Wynne for the purpose of an announced prelicensing change of ownership inspection. This is a change of ownership from Glory Children's Learning Center (#343621269). Applicant request an infant license to serve 14 infant children from birth to age two years. Census included one infant and one staff. The program will operate Monday through Friday from 6:30 AM to 5:30 PM. The fire clearance was granted on 10/12/2021.

Applicant acknowledges that the following documents must be posted at all times: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, Effects of Lead Exposure, car seat poster, menus, and daily schedule. AS and LPA provided and discussed LIC311A and the forms that must be in each child's and each staff member's file. Applicant stated the Needs and Services plans will be kept in the classrooms. Applicant stated parents will provide all formula, milk, and food for children under 1 and the program will provide provide breakfast, lunch and snacks for older infants.

INDOOR ACTIVITY SPACE:
There are two infant classrooms: Bright Babies and Early Explorers. Bright Babies will be designated for infants 0-12 months with a napping area and Early Explorers will be designated for infants ages 12-24 months. AS and LPA observed 4 cribs and seven napping cots. Applicant acknowledges there will be a maximum of 4 infants under 12 months old, due to available cribs. AS and LPA did not observed a separation of infant napping area from the activity space in the Bright Babies Classroom. Bright Babies Classroom is currently used as the napping room and the Early Explorers is used as the indoor activity space. Applicant was advised that until the dividers have been purchased and installed, Bright Babies cannot be used as an additional classroom.

AS and LPA observed a variety of age-appropriate toys and equipment. AS and LPA observed a sufficient number cubbies for infants and toddlers. Applicant was requested to purchased additional toddler chairs and tables. There is a first aid kit in each of the classrooms. Medications will be stored in the cabinets and inaccessible to children in the kitchen. AS and LPA observed cleaning disinfectants are appropriately stored and inaccessible to children. Applicant stated there are no firearms on the premises. Applicant stated drinking water for indoor and outdoor will be provided via water pitcher. AS and LPA observed a functional carbon monoxide detector in the lobby. AS and LPA observed a paper sign-in/sign-out system.

AS and LPA measured the classrooms. The total classroom space contains a total of 496.4326 square feet, which will accommodate Applicant's requested capacity for 14 infant children. There are four toilets and three sinks available to children, and a separate staff restroom is available. Individual measurements are recorded on the Capacity Worksheet (LIC 9024). AS and LPA observed a changing table with a changing pad that is at least one inch think with raised sides that are at least three inches tall in the Bright Babies Classroom. However, the changing table will need to be replaced due to instability. AS and LPA observed a second changing table in the restroom; however, the changing pad will need to be replaced. Applicant stated a second sink will be installed in the Early Explorers Classroom and an additional changing table will be purchased. Children who become ill during the day will be isolated in the office area and will use the staff restroom, if necessary.

OUTDOOR ACTIVITY SPACE:
There is one outdoor area on the property designated for preschool and is separate from the infant outdoor activity space. The infant and preschool outdoor activity space is currently separated by a plastic mesh safety net which is used as a temporary fence. Applicant stated she plans on putting a permanent fence in. The outdoor play area is fenced with wood fence that is four feet tall. AS and LPA observed a variety of age-appropriate outdoor toys and equipment. The playground is shaded by a patio cover; however the patio cover roofing will need to be replaced.

AS and LPA measured the outdoor activity space. The outdoor play area contains a total of 915.0889 square feet, which will not accommodate the applicant's requested capacity of 14 infants.

Applicant is to request an insufficient outdoor activity space waiver request where there will be no more than 12 infants at a given time.
AS discussed the following: 100% supervision is required at all times; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds. AS discussed with Applicant any changes that may occur regarding the director or an employee acting in the director's absence must be reported to department within 10 working days.

Prior to this inspection, AS emailed Effects of Lead Exposure brochure and Provider Notice Information (PIN) 21-21 (Release of the Written Directives for Lead Testing of Water in Licensed Child Care Centers per AB 2370). AS and LPA reviewed lead testing requirement and COVID-19 technical assistance with the applicant.



This facility plans to provide Incidental Medical Services - IMS. For IMS information, see Evaluator Manuel - Regulation Interpretations and Procedures for Child Care Centers Section 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage athttps://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Licensee was encouraged to visit the Department website at WWW.CCLD.CA.GOV for child care updates, current forms, legislation and regulation information. A copy of this report will remain on file for a period of three years for public review upon request.

A provisional license is granted today, 11/16/21 pending the following items:

1. Dividers to separate infant napping area from activity space in the Bright Babies Classroom
2. Full clean and sanitation of the entire facility
3. Replace changing table in the Bright Babies classroom
4. Fix sink and obtain a second changing table for Early Explorers
5. Infant cubbies for the younger infants
6. New changing mat in the bathroom
7. New trash can in the kitchen for solid waste
8. All poisons to be locked with a key pad lock or combination lock
9. Cleaning items to be separated from food items
10. Food items to be not expired, labeled with date that it was opened, labeled open boxes with expiration date and place food items in air-tight containers

12. Install fencing to separate infant play yard from preschool play yard
13. Replace roofing on patio deck in the infant yard
14. Ensure there are no nails or screws protruding from fencing
