Application Specialist (AS) Mai Lor, and Licensing Program Analyst (LPA) Josiah Gathing met with Zhen Zhen Li for the purpose of an announced pre-licensing change of ownership inspection. This is a change of ownership from My Preschool (#343623013). Applicant request a preschool license to serve 72 preschool children from age two to entry into first grade Census included 47 preschool and 6 staff. The program will operate Monday through Friday from 7:30 AM to 5:30 PM. The fire clearance was granted on 01/21/2022.

AS reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. Applicant acknowledges that the following documents must be posted at all times: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, Effects of Lead Exposure, car seat poster, menus, and daily schedule.

Applicant stated the facility will provide AM/PM snack. Parents are to provide lunch.

INDOOR ACTIVITY SPACE:
There are three classrooms and one gym room: Classroom #2, #3, and #4. AS and LPA observed sufficient amount of age- appropriate equipment, toys, and cubbies. AS and LPA’s did not observed sufficient mats/cots, chairs and tables to meet the requested capacity. AS and LPA observed the first aid kit to be incomplete. Applicant stated medications will stored in the office. Applicant stated there are no firearms or weapon on the premises. Applicant stated indoor and outdoor drinking water is provided via water pitchers with individualized water bottles. AS and LPA observed carbon monoxide detectors throughout theacility. AS and LPA observed an electronic sign-in/sign-out system.

AS used previous measurements from report date 6/24/2019. The total classroom space contains a total of 2992.16 square feet, which will accommodate the requested capacity for 72 preschool children. Two children bathrooms are located between classrooms. AS and LPA observed 5 toilets and 4 sinks in Bathroom #1 (Girls), and 3 urinals, 3 toilets and 4 sinks in Bathroom #2 (Boys). However, in Bathroom #1, AS and LPA observed two sinks to have low water pressure. In Bathroom #2, AS and LPA observed two non-working sinks and one broken toilet. Applicant was advised to fix the low pressure/non-working sinks and toilet. There is a separate private restroom for the staff. Children who become ill during the day will be isolated in the office and will use the staff restroom, if necessary.

OUTDOOR ACTIVITY SPACE:
There are three separate outdoor activity space for preschool. They are located at the front by the main street, in the back of the facility and in the parking lot. All three outdoor play area is surrounded by a either a mixed of wrought iron fence and masonary wall that is at least four feet tall. The fence by the main street has an approximately five inches of opening space between the concrete and the fence. The preschool playground in the back of the facility has a play structure that is anchored to the ground. AS and LPA did not observe a safety label. AS and LPA observed rubber used as cushioned under climbing structure. Applicant was reminded that children are to use age appropriate toys and equipment. Applicant acknowledges that supervision is to be provided at all times during outdoor times. AS and LPA observed sufficient amount of age appropriate equipment and toys. There are shaded areas supplied by trees and overhangs.

AS and LPA measured the outdoor spaces. The front outdoor play area by the main street contains a total of 263.0929 square feet. The playground located in the parking area contains a total of 759.2409 square feet. The outdoor play area located in the back of the building a total of 1674.143 square feet. The total outdoor space is 2696.477 square feet. This will only accommodate a maximum of 35 preschool children. Applicant request use the Gym as a supplement to the outdoor activity space. The Gym was previously measured on 6/24/19 at 837.41 square feet which will accommodate 11 preschool children. Applicant shall submit an insufficient outdoor waiver with a maximum of 35 children outdoor and a maximum of 11 children in the Gym with a rotating schedule. Individual measurements are recorded on the Capacity Worksheet (LIC 9024).

AS discussed the following: 100% supervision is required at all times; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds. AS discussed with Applicant any changes that may occur regarding the director or an employee acting in the director's absence must be reported to department within 10 working days.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. Applicant was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Licensee was encouraged to visit the Department website at WWW.CCLD.CA.GOV for child care updates, current forms, legislation and regulation information. A copy of this report will remain on file for a period of three years for public review upon request.

The following items are required before a license will be issued:

1. Revised fire clearance

2. Gap between concrete and start of the fence shall be enclosed

3. Two sinks in each of the bathrooms shall be fixed

4. Toilet in the Bathroom #2 (Boys’) shall be fixed

5. Submit insufficient outdoor waiver with a maximum of 35 children in the outdoor area

6. Submit waiver request to use the gym as a supplement for the outdoor space with a maximum of 11 children.

7. Proof of additional chairs, cots/mats, and tables to purchased

8. Proof of purchased of first aid kit

9. Parking lot outdoor space - water sprinklers need to be made inaccessible and uneven asphalt where the drainage pipe shall be constructed that is not a tripping hazard.
