32 | LPA observed 10 children with one teacher in room 1 and 15 children with one teacher in room two. Facility was out of ratio. Angela understands the conditions, limitations, and capacity specifications of the Facility license. Upon arrival, LPA observed one school age child (Child 5) walk from school age classroom to preschool classroom without supervision. Angela understands that children shall be visually supervised at all times. LPA observed that both rooms are clean. LPA observed the flooring is torn when entering the classroom. Angela states that the Facility staff rotate cleaning duties on a weekly basis at the Facility. Drinking water is readily available for the children in each room and in the outdoor playground area via water dispenser/pitchers/disposable cups. LPA observed solid waste containers with tight-fitting lids in the Facility. Staff and children's bathrooms are clean, sanitary, and operable. There is a separate staff toilet not utilized by the children which an isolated child can use if needed. Angela states that there are no weapons or firearms on the premises.
The food preparation and storage areas are clean, free of litter & rubbish, and free of rodents and other vermin. Angela states the facility does not provide snacks but have snacks available if a child does not have. Cleaning supplies are inaccessible to the children. Any poisons are stored in locked storage cabinets. Any medications at the Facility are stored in the director's office area. No IMS is administered by the Facility.
LPA observed lifted cement in the play area that can be a potential tripping hazard, a piece of wood on the railing of the stairs that is broken and can injure children, siding of building is coming off, a swing that is broken, outdoor chairs are cracked and cause a potential hazard and fence is falling apart. Several trees and the building overhang provide shade for the day care children. LPA did not observe any bodies of water.

The following type A and B deficiencies were cited on the attached page (809-D). Licensee was informed that failure to correct the deficiencies by the specified Plan of Correction (POC) Due Date may result in assessment of civil penalties in the amount of $100 per day per violation until the correction is made.
LPA Deanna Villagrana informed Director Angela Sanchez that this report dated 09/15/2022 document(s) 3 Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.
