On 08/02/2021 at 02:30pm Licensing Program Analyst (LPA) Justin Giese made an unannounced Case Management visit to the facility for the purpose of delivering an amended report. During today’s visit, LPA toured the facility and census was taken. LPA met with Christine Hanafi, Director to deliver amended report.

At time of visit no deficiencies were cited.

Exit interview conducted and a copy of this report and notice of site visit was provided to the Director. A copy of this report must be made available to the public for 3 years.
