On April 17, 2023 at 11:30 a.m., Licensing Program Analyst (LPA) Kendal Zirbes conducted an unannounced Annual Required Inspection and met with Administrator Sherry Kaufman. LPA disclosed the purpose of the inspection and was granted entry into the facility by the Administrator. 55 children and seven (7) staff members were present at the facility during the inspection. The preschool program operates out of six classroom, with a toddler component that operates out of classroom #1. The preschool center has six bathrooms, one staff bathroom, front office, teacher lounge and outdoor play space. The hours of operation: 7:00am to 6:00pm Monday to Friday.
Indoor/Children's Area:
Child care center was observed to be clean, safe, sanitary and in good repair; all outdoor and indoor passageways, stair ways, incline, ramps, open porches and other areas of potential hazard are kept free of obstruction; floors of all rooms have a surface that is safe and clean. Cleaning compounds were stored in cabinets equipped with magnetic locks or on high shelves that were inaccessible to the children in care. Per Administrator zero poisons are stored within the preschool. LPA observed the furniture/equipment is in good condition. Classrooms were free of flies, other insects and rodents. The classroom had tables and chairs to meet the children's needs. LPA observed the classroom had a designated carpet areas for story/instructional time, a variety of art supplies, dramatic play equipment, blocks, books and a wide variety of additional educational materials. Children's belongings are kept in cubbies labeled with the children's names. Trash cans for solid waste have tight fitting lids. Drinking water is readily available indoors via a gallons of water and personal water bottles brought by the families. All materials and surfaces are toxic free, window screens in good repair and free of debris. There is a working carbon monoxide detector, smoke detector and Fire Extinguishers located throughout the space. Each classroom has a bathroom with two toilets and one sink. . Toilets and sinks are functioning properly and are age appropriate. LPA observed soap, toilet paper and paper towels readily available. Classroom #1 and classroom #2 were equipped with diaper changing tables. Report continued on page two

Based on interview and record review, the licensee did not comply with the section cited above when two of two toddler files reviewed (C1,C2) were missing documentation of the 15 minute safe sleep checks as required. Interviews confirmed the 15 minutes checks were not being recorded for all children under 24 months. This poses/posed a potential health, safety or personal rights risk to persons in care.

There was working telephone service within the preschool. There is an isolation area for children who become ill. The facility maintains a comfortable temperature at all times. LPA observed the centers sign in/out sheets, which are completed daily. Smoking is prohibited on the premises. Teachers conduct daily inspection for illnesses. LPA did not observe prohibited childcare items during this inspection. Firearms/weapons are not allowed or stored on premises. LPA did not observe bodies of water on the premises. Two snacks are provided by the Center each day and the Parent provide their child with lunch. The snack menus were posted in the classrooms.
Napping: LPA observed the children sleeping on cots. The cots were in good repair.
Outdoor: LPA inspected the outdoor play space for health, safety, cushioning material, good repair and age appropriateness. The Center has two play grounds, one for the children ages three and above and the other play space is for younger children. The outdoor activity space is fully enclosed and has shaded areas. The area had one large climbing structure, swings, tiny tikes slides, and two sand boxes. The climbing structures were placed on sand and were secured to the ground. LPA observed the wooden barriers surrounding the sand boxes had multiple areas where paint was starting to peel. LPA advised the Center to repaint an areas that were showing signs of wear with the paint peeling away. LPA also advised the Center to wrap the wooden vertical and horizontal beams surrounding the covered sand box in order to prevent potential injury. Water is accessible, via personal water bottles provided by the families.

Health Related Services
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226.The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm. LPA observed one child file did not contain in writing, approval and instructions from the child's authorized representative for the administration of the medication. Per interview with the Administrator, the medication has not been provided to the child. LPA advised the Center that written authorization must be provided by the authorized representative. An advisory notice was provided.

Child/Staff/Personnel Records
LPA reviewed 12 child records and five staff files during this inspection. Three of twelve child files reviewed were missing the parent signature at the bottom of the LIC 627. One of twelve child files reviewed was missing the written medical assessment. This child has been enrolled for over 30 days. Advisory notices were issued. Per interview with Administrator and LPA reviewed of Child 1 (C1) and Child 2 (C2) files, the Center is not documenting the 15 minute safe sleep checks which are required for all children under 24 months of age. A type B citation was issued, as this is a potential risk to the children in care. Staff records were complete and contained all required information.

Facility Records


LPA and facility representative reviewed the LIC 311A Records To Be Maintained At The Facility. LPA observed a current roster, fire/disaster drill log completed 2022. An staff member was present who was current on CPR/First Aid.
Posting Requirements
The following were observed posted as required: facility license, Personal Rights (LIC613A), Parent’s Rights Poster (PUB 393), menu, car seat law (PUB 269), emergency disaster plan, earthquake preparedness checklist. The current posted disaster plan was updated in 2017. LPA advised the Center to update the disaster plan and ensure all staff assignments are assigned to current staff members. LPA also advised the center to post the phone number to the local health department.
Other: Electrical outlets are inaccessible, there are no recalled and or prohibited toys or sleep/play equipment observed on the premises.

Any duly authorized officer, employee, or agent of the Department shall, upon presentation of proper identification, shall inspect the facility. The Center shall permit the Department to inspect the child care center and to privately interview children or staff, to determine compliance with or to prevent violations of child care laws or regulations, also enter and inspect any place providing personal care, supervision and services at any time, with or without advance notice, to secure compliance with, or to prevent a violation.

Facility representative was advised of the requirement to report Unusual Incidents. Facility representative informed to utilize the Unusual Incident Report/Injury Report LIC624 when submitting the report to the department (email address: unusualincidentreport@dss.ca.gov). A report shall be made to the department by telephone or fax during the department's normal business hours before the close of the next working day following the occurrence during the operation of center. In addition, a written report shall be submitted to the department within seven days following the occurrence of any events specified above. The On-Duty Worker is available for questions at (661) 202-3318 Monday through Friday 8am-5pm.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The Center received Lead testing in January 2020.
