1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	Licensing Program Analyst (LPA) Zirbes met with Facility Representative Sherry Rice (Kaufman) for the purpose of conducting an unannounced Annual/Random inspection for the Preschool Center. During the inspection, LPA observed 10 toddlers with one teacher and one aide providing supervision and 29 preschool age children with five teachers, one aide with additional Administrative and support staff available. Based on LPA observation the Center was meeting the ratio and capacity regulations during this inspection. The preschool age component is operated out of five classrooms (2, 3, 4, 5, 6) and the toddler component is operated out of classroom 1. The Center is also licensed to provide care to school age children. The Center has a kitchen, staff restrooms, staff office and outdoor activity space which are utilized by both components. The facility currently operates Monday through Friday 7:00am -6:00pm. Indoor/Children's Area: Child care center was observed to be clean, sanitary, and in good repair; all outdoor and indoor passageways, stair ways, incline, ramps, open porches and other areas of potential hazard are kept free of obstruction; floors of all rooms have a surface that is safe and clean. Cleaning compounds were stored in each classroom on a high inaccessible shelf. LPA observed the indoor furniture/equipment was in good condition. Classrooms were free of flies, other insects and rodents. The classrooms had tables and chairs to meet the children's needs. LPA observed each classroom had a variety of art supplies, blocks, books, dramatic play, library space. Children's belongings are kept in cubbies labeled with the children's names. Trash cans for solid waste have tight fitting lids. Drinking water is readily available indoors via children's water bottles. Children are dropped off with a full water bottle and if needed the water bottled is refilled with bottled water. All materials and surfaces are toxic free, window screens in good repair and free of debris. LPA observed carbon monoxide detectors throughout the licensed space. Report continued on page two

Restrooms: Each classroom has a restroom with two toilets, and two sinks. Classroom 1, 2 each have a changing table the other diaper changing pads. Toilets and sinks were functioning properly, sanitary and are age appropriate. Paper towels and soap were available to the children.
Meals: The families of the enrolled children provide lunch and the center provides two snacks. The kitchen areas was clean and sanitary at the time of this inspection.
Napping: Mats are utilized for napping.

Outdoor: LPA inspected the outdoor play space for health, safety, cushioning material, good repair and age appropriateness. The space contained two outdoor activity spaces one for the toddlers and a second for the preschool/school age children. The preschool area had a climbing structures. The climbing structure was placed on sand and was anchored to the ground. LPA observed the paint on the wooden borders surrounding the sand boxes in both areas of the outdoor activity spaces had areas with chipped paint. One wood board had an approximate two foot section with exposed wood. When LPA touched the wood, the wood was rough and was a potential safety hazard to the toddler and preschool age children. Administrator could not recall the last time the border had been painted. Administrator confirmed the children play in the sand box areas and expressed to this date a child has not sustained an injury due to the rough wood edges. The Center was issued an advisory notice in 2023 regarding this same observation. A type B citation was issued for regulation, 101238.2(d) (2), as the peeling paint and exposed rough wood is a potential hazard to the children in care. Refer to 809D

Facility Records
Per LPA record review, the last emergency drill occurred in October of 2023. When asked about the emergency drills the Administrator stated an emergency drill occurred in Spring of 2024, however the drill was not documented. LPA and Administrator discussed the disaster drill requirements. A advisory notice was issued.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).
LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Per LPA record child 2 (C2) had a prescription medication at the Center with an expiration date of July 2024. Records revealed C2 was enrolled in June 2024. Per conversation with the administrator, the child has not been administered the expired medications, this was confirmed through facility records. Administrator stated the initial review policy for prescription medications was not followed according to the Centers policy. LPA advised the facility representative to ensure the Centers incidental medical policy is followed. A technical advisory notice was issued.

Due to time constraints LPA was unable to complete inspection. LPA will return on a future date to continue child and staff record reviews.
As a result of this inspection, one type B citation and two advisory notices were issued.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative Sherry Rice (Kaufman).
