On 09/25/2023 at 1:54 PM A required – 1 year inspection is being conducted as part of a compliance review. Licensing Program Analyst (LPA) Susan Brewer, met with Site Director Sherry Riley, toured the school-age center, inside and out. The following was observed: Temporary outdoor activity is on the East end of the school. The licensee agrees to submit a request for a waiver with activity schedule, to operate on the East outdoor playground, which is shared with another afterschool program serving elementary school children. Area Director Sandra Robles, to join the inspection.

Days and Hours of Operation in Room B1: Mornings Mon-Fri, 7:00 AM to 8:30 PM
Afternoons: Mon/Tue/Thu/Fri, 2:00 AM to 6:30 PM and Wed, 1:30 PM-6:30 PM.
Staff are Present Mon-Fri mornings from 6:30 AM to 8:30 AM and Mon/Tue/Thu/Fri afternoons, from 1:30 PM to 6:30 PM and on Wed from 12:30 PM-6:30 PM.

The inspection consisted of reviews of the following domains: Food Service Reporting Requirements
Physical Plant, Care and Supervision Children Records, Staff Records Staffing Ratio and Capacity
Personal Rights. The inspection found the facility to be in compliance in these domains, except as noted on the LIC809-D. A review of the staff records and children's records were conducted as part of this evaluation. The licensee is asked to update the following documents and submit to licensing within 30 days: Only if changes have been made:
1. LIC 500 Personnel Report.
2. LIC 610 Emergency & Disaster Plan
3. Parent Handbook/ Program Curriculum/Admission policies and procedures/ fee schedule (only if changes have been made)

The following items have been posted and are updated where necessary:
- License, Emergency Disaster Plan (LIC610) and Earthquake Preparedness Checklist (LIC9148)

· The facility is operating within the terms of the license
· Facility is clean, safe and sanitary and in good repair.
· Ratios were met during this inspection. LPA took a census of 25 children supervised by 3 staff.
· A fully charged fire extinguisher (2A:10BC) was observed, inspected on 12/23/2022 by the fire department. A smoke detector and carbon monoxide detector were present and tested by the Licensee during this inspection 09/25/2023.
· Appropriate supervision was provided during this inspection.
· Classrooms are equipped with age-appropriate furniture and equipment in good condition.
· Classrooms are clean and free of hazards.
· All materials and surface accessible to children are toxic free.
· No weapons stored at the facility.
· There are no bodies of water present. All wading pools or similar product must be emptied immediately after use and stored in an upright position.
· Medications are stored where inaccessible to children. No medications are stored at this time.
· Center is equipped to isolate and care for any child who becomes ill during the day.
· Hazards are stored where inaccessible to children which include: disinfectants, cleaning solutions and other items that are dangerous to children.
· All floors shall be clean and safe in room B1.
· Measures are taken to keep the facility free of flies, other insects and rodents.
· Toxins are locked.
· Bathrooms were observed to be hazardous due to sitting water on the floor, un-sanitary and not in operating condition. During the inspection of the girls designated restroom, LPA S. Brewer observed 3 of the 5 restroom stalls to be flooding with water. In addition, 1 of the 5 toilets inspected did not operate. All 3 sinks in the designated restroom were inspection and observed to be operating efficiently. A school employee stated they have reported the equipment to the school site. LPA inspected 1 sink and 1 toilet designated for boys, which were operable.

· Playgrounds are enclosed by appropriate fences and free of hazards. Per site supervisor Sherry Riley, they program has not used the designated outdoor playground on the West end of the school as requested in 06/2009.
· Outdoor activity areas are supplied with age and size appropriate equipment in good condition. LPA did not observe any shade equipment or working water fountains in the West playground, adjacent to the Room B1. Per S. Riley, the program has been utilizing the Large Playground on the East side of the school due to the West yard, due to the West outdoor play area not having shade or working water fountains.
· Food preparation area is clean and free of vermin.
· Food is stored appropriately and protected from contamination.
· All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair on 09/25/2023
· Uncontaminated drinking water shall be readily available both indoors and out and is provided by bottled water served in disposable cups and outdoor water fountains.
· The areas around or under high climbing equipment, swings, slides, and similar equipment shall be cushioned with material that absorbs a fall wood chips and natural grass.
· Menus shall be posted at least one week in advance in a place visible by the child’s authorized representative, dated and kept on file for 30 days, and made available upon request.
· Sign in/Sign out record was reviewed and meets regulation requirements.
· A Staff member is present with current Pediatric CPR/First Aid which expires on 02/2024
· Opening and closing staff member’s CPR/First Aid expires on 12/2024
· Director completed Health and Safety Training on 02/19/2023.
· Staff qualifications and files were reviewed – health screening is on file and all staff meet educational requirements and health requirements for performing assigned tasks
· Staff received on the job training for house keeping, sanitation and universal health precautions
· Each child’s file is complete on 09/25/2023.
· Licensee was informed of the Department has inspection authority per Health and Safety Codes
· Documentation of Lead Testing is on file: NOT ON FILE, per director Sandra Robles, the facility is in the process of scheduling, however a date for testing has not been secured at this time.

· A review of staff records on 09/18/2023 and 09/25/2023 indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
The Licensee was informed of their reporting requirements and is provided with the Regional Office’s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO09@dss.ca.gov

This facility provides Incidental Medical Services – IMS. No children currently receiving IMS services, however LPA S. Brewer reviewed storage area of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Site Director Sherry Riley, was reminded that all adults 18 and over living or working in the facility, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations862@dss.ca.gov for Riverside Regional Office.
