25 | On 3/25/2022 at 8:15 am, Licensing Program Analyst (LPA) Patricia Berry conducted a case management incident investigation. LPA met with director Melinda Gaskin and was granted access into the facility. LPA toured the facility and took a census.
On 3/7/22 it was self reported by director a child was running toward the sink to wash hands. Child tripped, and fell hitting child’s forehead on the edge of the sink. Director stated the child received stiches on the forehead. LPA observed the area where the child fell. LPA observed a mat on the floor at the sink area. LPA observed the mat to be in good condition with no apparent tripping hazards. LPA also observed there were no other obstructions at the sink area. LPA interviewed staff who stated they were in transition, coming from the playground into the classroom for lunch. Staff stated child was running to the sink to wash hands, when child tripped over child's feet, fell forward and hit the edge of the sink. LPA interviewed the child and child revealed child was running toward the sink, tripped and fell. Child stated there was nothing by the sink except the mat on the floor.
Based on information obtained, and interviews with staff and child there did not appear there was any obstruction in the way to cause the fall. LPA observed the mat only at the sink area. Therefore, there has been no violation of Title 22 regulations at this time. This incident appears to have been an accident.
