Staff stated the authorized representative is notified verbally when their child is sick and are asked to pick up their child. LPA reviewed the sick policy updated version 2/2023 and it states "the child will be removed from the classroom upon notice and kept in an isolated area where there will be no direct contact from other children". The parent will be contacted and expected to pick up child within 1 hour of notification". Director stated all communications with sick children are verbal notifications with authorized representatives. LPA requested documentation from July 2020 to review children's files regarding the allegation. Director was not able to produce children's files from July of 2020 to determine if there were any communications with parents regarding isolating sick children; therefore, there is conflicting information. Based on information obtained, there was no documentation from children's files to determine if sick children are isolated, although the allegation may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, the above allegation is unsubstantiated.

Lack of Supervision resulting in a child injury. Staff stated when an injury occurs, the director is notified, and parents are called. Staff stated an incident report is given to the parents after each injury. LPA reviewed policy updated version 2/2023 and it states "parent receives communication regarding accidents and injury. "if your child is injured at the center you will receive a incident/accident report at pick-up time". " If the injury needs medical care the center will make every effort to contact you and we will make sure your child receives medical treatment and will call all contacts listed on the enrollment agreement". LPA requested documentation from July 2020 to review children's files regarding the allegation. Director was not able to produce children's files from July of 2020 to determine if there were any written communications with parents on child's injury. Director stated injury reports from 2020 are not available due to length of time; therefore, there is conflicting information. Based on information obtained, there was no documentation from children's files to determine if a parent was notified of an injury, although the allegation may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, the above allegation is unsubstantiated.

Exit interview conducted with director, report, appeal rights and notice of site visit issued. Notice of Site visit must be posted for 30 days.
