· Poisons and toxins are locked
· All floors are clean and safe
· Bathrooms were observed to be safe, sanitary and in operating condition
· Playgrounds are enclosed by appropriate fences. The area where the drain is located presents a drop in the grass area. Director agreed to barricade the drain area to prevent tripping or injuries. Picture will be submitted to the Department by 5/6/2019.
· Outdoor activity areas are supplied with age and size appropriate equipment in good condition
· Food preparation area is clean, free of litter, rubbish and free of rodents and other vermin
· Food is stored appropriately and protected from contamination
· All storage containers for solid waste, including moveable bins have covers that are kept on, and in good repair
· Menu for the current month is posted
· Drinking water is readily available both indoors and out and provided by use of the drinking faucets and drinking fountains
· The areas around or under the play structure are cushioned with rubber filler that absorb falls
· Sign in/Sign out record was reviewed and meets regulation requirements
· A Staff member is present with current Pediatric CPR/First Aid which expires in January 2021
· Opening and closing staff member’s CPR/First Aid expires in January 2021 (Opening teacher Crystal Porras and closing teacher Guadalupe Lugo)
· Required records for children shall ensure that each child’s record contain a medical assessment and contain the Identification and Emergency Information
· Required records for staff shall ensure that each personnel record contain a health screening
· Documentation of fire & earthquake drills was reviewed. The most recent fire drill was conducted on 4/1/19 and earthquake drill on 4/11/19
· A review of staff records on 5/2/2019 indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
This facility provides Incidental Medical Services – IMS. Plan of operation including IMS is on file. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.
