25 | On 03/01/2022, Licensing Program Analyst (LPA) Destinee Hogue conducted a case management inspection with Director Patricia Arth. A case management inspection is being conducted in response to the receipt of an unusual incident report (UIR) received from the facility. The UIR was received by the licensing agency on 11/29/2021. On 11/30/2021, an inspection was conducted by LPA Hogue. During the 11/30/2021 inspection, LPA Hogue interviewed pertinent parties, reviewed facility records, and conducted facility observations. During this inspection, LPA Hogue toured the facility inside and outside, took census of preschool children present on this date, verified facility associations, and discussed the following with Director Patricia Arth.
On or about 11/17/2021, a child’s authorized representative reported to the facility that their child(ren) disclosed being touched in the anus by another child(ren) and staff member at the facility. According to the reported UIR, the child(ren) did not disclose the name of the suspected child(ren) and the suspected staff member has not worked at the facility since September 2021.
LPA Hogue interviewed staff members working at the facility on or about 11/17/2021 and there were no disclosures of staff members observing a child(ren) touching another child(ren)’s anus or touching a child(ren) inappropriately. Staff members interviewed decline inappropriately touching a child(ren)’s anus. According to interviews, staff members in Classroom #1 do diaper changes for children who are not potty trained. Based on interviews, the victim child(ren) is fully potty trained, does not wear diapers, and does not require assistance when using the bathroom. LPA Hogue reviewed records from involved outside agencies and based on records reviewed, the victim child(ren) did not disclose the name of the suspected child(ren) or suspected staff member and did not disclose details about being touched inappropriately by a child(ren) or staff member.
At this time, based on information gathered, there appears to be no violations of Title 22 Regulations found. An exit interview was conducted, and a copy of this report was provided to Director Patricia Arth. No deficiencies were cited during this inspection and a Notice of Site Visit was issued and shall remain posted for 30 days. |
