On this date, LPA Zuniga also concluded a complaint investigation which was found Unsubstantiated. It was alleged that a child received injury (scratches on face) while in care. During the investigation, LPA was informed that when the guardian concern was brought up to the center staff, management made a decision to suspend the child's care. The child did not have any behavioral concerns. This is a violation of the center plan of operation handbook because the center did not operate within its own guidelines.

An exit interview was conducted and during the interview, director confirmed that there are no Registered Sex Offenders living in the facility and/or using the facility address for their mailing address. |
