On March 28, 2023, Licensing Program Analyst (LPA) Babatunde Ibitoye met with Site Supervisor Patricia Rosales to conduct an unannounced case management inspection. The purpose of the case management was to follow up on a self reported unusual incident report (UIR) submitted to the Department on January 12, 2023.

Description of incident: On 01/11/2022, around 4:20, Site supervisor was notified by Teacher #1 regarding Site custodian picking up a child #1 by arm. Site custodian heard a child #1 crying and went to classroom 90 to assist . As he walked over, C # 1threw himself on the floor, and site Custodian reached down with his right hand and pick up C#1 by his wrist. He walked with C#1 to the library and comforted him until he was calmed and continued his activity.

Present during the time of this inspection is the site supervisor, teacher #1,Aide #1 The copy of the facility roster, sign-in and out sheet for the incident day was collected.

Further investigation is needed, An exit interview was conducted, and a copy of this report was read and provided to the site supervisor Patricia Rosales along with a Notice of the Site Visit
