25 | On 03/26/2024 at 11:10 a.m. Licensing Program Analyst (LPA) Blanca Ruiz arrived at the facility to conduct a case management inspection to reassign classroom 5 as a Toddler classroom and to verify substantial compliance with Title 22 regulations. LPA was greeted by facility administrator Agatha Kolenda, and director, Laura O’Brien. LPA toured the facility, inside and out, reviewed records, and observed and/or discussed the following:
Normal days and hours of operation are Monday- Friday, 6:30 a.m. to 6:00 p.m. for Preschool classrooms: Rooms 1-4, and 6-8 and Toddler Classroom, Room 5 from 7:00 a.m. to 5:00 p.m.
During inspection the following documents were requested to complete by licensee:
1. Please updated Lic 200A -Correction needed.
2. Parent Handbook/ Program Curriculum/Admission policies and procedures/ fee schedule for Toddler Option Component
3. Facility Sketch Update- Playground area ( include Toddler Option Component)
4. Napping waiver -Activity area
5. Proof of purchase and/ or pictures of required classroom equipment obtained-Potty Chair(s). Obtained during inspection.
6. LIC 500 Personnel Report- Staff/ Toddler Option Component-Academic requirements.
7. Fire Department follow up due to changes in the playground exit(s)/ due to capacity rearranges.
Once all corrections have been verified, the request will be submitted for approval. A NOTICE OF SITE VISIT WAS GIVEN AND MUST REMAIN POSTED FOR 30 DAYS. Exit interview conducted and report was reviewed with facility, Director Laura O'Brien. |
