On January 25, 2024, Licensing Program Analyst (LPA) Zirbes met with the Administrator Lisa Baldwin for the purpose of conducting an unannounced required one year inspection. LPA toured and inspected the facility in accordance with the facility sketch. There were 12 school age children, one teacher and administrative staff on site. Based on LPAs observation the facility was meeting the capacity/ratio requirements during this inspection. The school age program is operated out of two classrooms (B4, B5). The center is located on Valley Community Chapel Campus. Therefore the Center utilizes the Valley Community Chapel's kitchen, office spaces and staff bathrooms located in the Social Hall. The center also operates a Preschool Program (364813646). The facility operates Monday through Friday 6:00am -6:00pm.
Indoor/Children's Area:
Child care center was observed to be clean, safe, sanitary and in good repair; all outdoor and indoor passageways, stair ways, incline, ramps, open porches and other areas of potential hazard are kept free of obstruction; floors of all rooms have a surface that is safe and clean. Cleaning compounds were stored in a key locked cabinet in B5. Per Administrator, zero poisons are stored within the licensed spaces. LPA observed the furniture/equipment was in good condition. Classrooms were free of flies, other insects and rodents. The classrooms had tables and chairs to meet the children's needs. LPA observed the Center has a variety of art supplies, dramatic play equipment, library, board games, foosball table and additional materials for the children to engage in play. Children's belongings are kept in cubbies labeled with the children's names. Trash cans for solid waste have tight fitting lids. Drinking water is readily available indoors via personal water bottles provided by the authorized representatives and water coolers/cups. All materials and surfaces are toxic free, window screens are in good repair and free of debris. There are carbon monoxide detectors, smoke detectors, a fire sprinkler system and multiple fire extinguishers located throughout in the classrooms. There was working telephone service within the classroom. There is an isolation area for children who become ill. The facility maintains a comfortable temperature at all times. LPA observed the centers electronic sign in/out sheets, which are completed daily.

Indoor/Children's Area continued: Smoking is prohibited on the premises. Teachers conduct daily inspection for illnesses. LPA did not observe prohibited childcare items during this inspection. Firearms/weapons are not allowed or stored on premises. LPA did not observe bodies of water on the premises.
Restrooms: The Center has a total of two children restrooms located in B4. Each restroom was equipped with one toilet and one sink. The restrooms provided the school age children with privacy. Restrooms were observed to be in safe and sanitary operating condition. Toilets and sinks are functioning properly and are age appropriate. Paper towels and soap were available to the children.
Meals: The Center provides three snacks a day during full day programs and two snacks when school is in session. The snacks are stored in each classroom. Menus were posted in the classrooms. Each family provides lunch for their child. The kitchen is shared with the Valley Community Chapel Campus
and is located in the off limits social hall.
Napping: The school age children do not nap at the Center however are provided with opportunities for quit play when needed.
Outdoor
LPA inspected the outdoor play space for health, safety, cushioning material, good repair and age appropriateness. The School Age component has two outdoor play spaces. All outdoor spaces are enclosed by a chain link fence. One space was located near classroom B4 and B5. The area consists of dirt, cement and artificial grass. The area is utilized for large motor activities. There are three locked storage sheds in the area which held additional school supplies, janitorial supplies. LPA observed the addition play space is a large blacktop area. LPA observed a tether ball court, sand box and dramatic play house. An approved waiver is on file allowing for the school age and preschool components to share playground space and co-mingle. Water is accessible to the children via drinking fountain and water coolers/cups and personal water bottles.
Child/Personnel Records
LPA reviewed five child records and five personnel files during this inspection. Per LPAs review child an personnel records were complete and contained all required information.

Facility Records
LPA and Facility representative reviewed the LIC 311A Records To Be Maintained At The Facility. LPA observed the disaster drill log (last drill completed in September 2023), sign in/out records. The Center had the required lead testing completed in 1.25.23.
Incidental Medical Services: This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Posting Requirements
The following were observed posted as required: facility license, waivers, Personal Rights (LIC613A), Parent’s Rights Poster (PUB 393L), menu, car seat law (PUB 269), emergency disaster plan.

Any duly authorized officer, employee, or agent of the Department shall, upon presentation of proper identification, shall inspect the facility. The Director shall permit the Department to inspect the center, and to privately interview children or staff, to determine compliance with or to prevent violations of laws or regulations, also enter and inspect any place providing personal care, supervision and services at any time, with or without advance notice, to secure compliance with, or to prevent a violation.

Facility representative was advised of the requirement to report Unusual Incidents. Facility representative was informed to utilize the Unusual Incident Report/Injury Report LIC624 when submitting the report to the department (email address: unusualincidentreport@dss.ca.gov). A report shall be made to the department by telephone or fax during the department's normal business hours before the close of the next working day following the occurrence during the operation of center. In addition, a written report shall be submitted to the department within seven days following the occurrence of any events specified above. Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

As a result of this inspection, zero citations were issued.
