Licensing Program Analyst (LPA), Babatunde Ibitoye met with Lead Teacher, Adriana Mandez today for the purpose of conducting a Required Annual inspection. This program occupies Rm #'s 2, & 3. Teacher/Child ratio's were observed: Rm#2 - 21 pre-school Children with 4 Staff; Rm #3 - 22 pre-school children with 4 Staff; LPA toured both classrooms and the Indoor/outside play area. This site has two sessions (morning and afternoon session). The hours of operation for this site range from 8:00 am to 8:15 am and dismal begins at 11:00 to 11:15 am for the morning session and for the afternoon session 12:00 to 12:15 pm for arrivals to 3:00 pm to 3:15 dismissals. Due to half day sessions, children do not nap at this site. Staff are fingerprinted through San Bernardino School District, staff files are also kept at the District Office. There is a waiver on file for swimming pool located within the same building which is not immediately accessible to children.

**LPA observed age appropriate furniture, equipment, toys and materials. The classrooms were observed to be clean and free of any Health or safety hazards. Telephone service was verified as well as adequate heating, lighting, and ventilation. Children's belongings are kept in cubbies along the wall as you enter classrooms. Drinking water is available inside the classroom in the form of a water fountain. Lead Testing was conducted on 2/24/23 all levels were within acceptable limits, less than 5 ppb.

**The children's bathrooms are located within the building, there is a bathroom for classroom #3 that has 2 toilets/2 sinks/1 urinal; LPA observed the bathrooms to be clean and sanitary, with soap, toilet paper and paper towels readily available. All toilets and sinks were functioning properly during this inspection.

**Outdoor play equipment was inspected for health, safety, good repair and age appropriateness. Outside Drinking water is available in the form of a water fountain. There is now rubber matting underneath jungle gym. The playground is enclosed by a fence to protect children and to keep them in the outdoor activity area.

**Each classroom has a Parent Board with required licensing forms. A sampling of Children's records were reviewed as part of this inspection. Center utilizes care connect App for signatures. LPA observed a fully stocked first aid kit; fully charged fire extinguishers; carbon monoxide detectors throughout the center. Fire Drill/Earthquake log is posted. The last drill conducted (2/9/2025) Staff are certified in Pediatric CPR and First Aid (exp 8/20/2025).

Posting Requirements :Failure to comply with posting requirements shall result in an immediate civil penalty. The following were observed posted as required: facility license, Personal Rights (LIC613A), Parent’s Rights Poster (PUB 393L), menu, car seat law (PUB 269), emergency disaster plan, earthquake preparedness checklist.

Food preparation: Center serves breakfast for AM sessions and lunch for PM classes delivered daily from the school cafeteria. menus observed and posted. Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children were stored and inaccessible to children.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Lead Teacher advised of the requirement to report Unusual Incidents. Licensee informed to utilize the Unusual Incident Report/Injury Report LIC624B when submitting the report to the department (email address on the website: www.unusualincidentreport@dss.ca.gov). A report shall be made to the department by telephone or fax during the department's normal business hours before the close of the next working day following the occurrence during the operation of family day care home. In addition, a written report shall be submitted to the department within seven days following the occurrence of any events specified above.

HEALTH RELATED SERVICES: Medications inaccessible to children; all prescription and non prescription medications must have child’s name and are dated; written consent and instruction from child’s representative; a plan to document and report to child’s representative when medication is administered to a child; IMS plan on file (if applicable).

Immunization Requirements: §1596.7995 (a)(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day-care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year.

Fingerprint clearances and transfers: Before working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review shall obtain a clearance or criminal record exemption. If a fingerprint clearance has been obtained through the Department, Licensee may request a transfer of a criminal record clearance from one state licensed facility to another using form LIC 9184



On January 1, 2018, or before March 30, 2018, a person who is a licensed child care provider, administrator, or employee of a licensed child day care facility shall complete the online mandated reporter training and shall complete renewal mandated reporter training every two years. @www.mandatedreporterca.com

· **Senate Bill AB 633 - Child Care Facilities: Parent Notification Requirements· Summary: This bill amends · Health and Safety Code (HSC) sections 1596.859, 1596.8595, 1596.8895, and 1597.05 to improve the transparency of licensing records and to ensure that parents/guardians using a licensed childcare facility are aware of situations that present the greatest danger to children.

· Each report (documenting a Type A citation) shall remain posted for 30 days along with the Notice of Site Visit. **In addition; A copy of this report must be provided to the authorized representatives of all currently enrolled children and any newly enrolled child for the following 12 months.

· The ACKNOWLEDGEMENT OF RECEIPT OF LICENSING REPORTS (LIC9224) shall be signed and kept in each of the children’s records. The report shall be provided no later than the next business day or the next day the child is in care.

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Director shall permit the Department to inspect the family child care home, and to privately interview children or staff, to determine compliance with or to prevent violations of child care center or regulations, also enter and inspect any place providing personal care, supervision and services at any time, with or without advance notice, to secure compliance with, or to prevent a violation.
