On 7/29/2021 at about 01:30 P M., Licensing Program Analyst (LPA) Mabika was greeted by above facility’s licensee, Jessica Flores. LPA was at the facility to conduct an unannounced random annual inspection. LPA disclosed the purpose of the visit and was granted entry by licensee, who guided LPA on a tour of the facility. Upon arrival, LPA observed eight children in the care of Licensee and 2 helpers. One staff and children were all napping except for one who was doing table activities in the living room. No Covid signs were posted outside Licensee states that they were blown away by the wind and will be replaced. Covid protocols were discussed including masking as upon arrival none of the staff and children were wearing masks..

The facility's hours of operation are Monday through Friday 6:00 AM. to 6:00 PM. The licensee provides 2 meals and 2 snacks. Licensee states she is not enrolled in a food program. There are 5 pet dogs on the premises and Licensee states that they have the current shots. The dogs are primarily kept in the backyard which is off-limits to children in care. The frontyard is used for outside play. The off limit areas include the bedrooms, two of the three bathrooms, the laundry room, and the attached garage and gates were observed.

The home was inspected inside and out for safety, comfort, cleanliness, telephone service, heating and ventilation, inaccessibility to poisons, detergents, cleaning compounds, medicines, and hazardous items that can pose a danger to children. Per the licensee there are no weapons or firearms of any kind in the facility at this time. Licensee stores medications in the off-limits bedroom and sharps are stored in a kitchen above a high cabinet.

The outside front yard is used by the children for outside play, and is brick walled and gated all around. The outdoor play area was inspected and play equipment was observed to be free of hazards, loose, or sharp part. LPA observed a brick walled patio with bikes, slide sets and other age-appropriate toys. LPA recommended that two of the bikes be either replaced or repaired for safety as the rubber pedals broke off There is a famly swimming pool in the off-limits backyard that is fenced.


The facility’s fire extinguisher (2A10BC) met the State Fire Marshal standards with current service tags.

LPA tested the smoke and carbon monoxide detector and found it to be in operable condition. The facility annual fees are current. The parent board was reviewed and had all of the required forms posted and accessible to parents. Facility looked clean and very orderly, in general.

Two Staff files were reviewed and found to be incomplete,


The facility roster had more than 20 children, LPA reviewed 3 children's files. Fire drills were not up to date (last recorded drill was dated 01/10/2020. Licensee stated it had been difficult with Covid and she was going to resume the following day), and all individuals living the family child care home have been fingerprinted and associated to the facility. Three assistant had current CPR and Pediatric First Aid certifications, with a renewal date of 10/2021. Licensee/Staff has proof of being immunized against influenza, pertussis and measles. Licensee stated she no more does transportation due to Covid and apart from that there was a bus that stops right by the curb.
Incidental Medical Services (IMS) policy was discussed. Licensee does not have any children needing IMS services currently. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following were discussed:
There is an effective 24/7 ban on smoking tobacco in a home that is licensed as a family day care home, and in those areas of the family day care home where children are present. Baby walkers, baby jumpers, baby exersaucers, baby bouncers/rockers and any other item that falls into that category are not permitted in the facility. The LPA also discussed earthquake safety and necessity of drills, required forms for children’s files, facility files and posting requirements. LPA also provided licensee with the new 2019 Safe Sleep in Child Care brochure.
The licensee was informed that all adults living in or having access to the home are required to have fingerprint clearances with Department of Justice, FBI and Child Abuse Index prior to having contact with children. If the aforementioned is not adhered to, a Civil Penalty of up to $500, per non-cleared adult will be assessed immediately. Please advise your analyst of any person who will be visiting regularly or for longer than one week.

Beginning on January 1, 2018, Assembly Bill 1207 (2015) requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Applicants must meet requirements as a precondition to licensure. New employees shall have 90 days from date of employment to complete training as required. The training may be conducted at the following website www.mandatedreporterca.com. LPA observed that 3 of the staff reviewed had completed this training on 12/12/2019, 8/27/2020 and on 3/31/20 respectively.

The facility was cited a "B" deficiency today for the Fire Drills in accordance with Title 22 regulations.
