On November 19, 2024, Licensing Program Analyst (LPA), Calloway conducted an unannounced case management inspection to the above facility. The purpose of the inspection was to conduct a follow up to an usual incident that occurred on October 23, 2024, that was reported to the Palmdale Regional Office. LPA met with the facility representative who granted access. The facility director was not on the premises when LPA arrived. LPA toured the facility with the representative and observed thirteen (13) school aged children and one representative in care during the inspection.

The Director reported an Unusual Incident Report (UIR) on October 24, 2024, Staff 1 inappropriately grabbed Child 1 during class time.

LPA conducted confidential interviews. Based on interviews conducted, there was no witnesses that observed the incident and there is not enough evidence to prove the incident occurred. Per Title 22 regulations, there are no deficiencies cited during this inspection.

Exit interview was conducted and a copy of this report was read, a Notice of Site Visit, were provided to Joseph Nettleton, Facility Representative. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.
