Licensing Program Analyst (LPA) Babatunde Ibitoye met with Lead Teacher, Ms Julie Phillipson today for the purpose of conducting a Required 1 year health and safety inspection. This State Pre-School is located on the grounds of Heritage School in Classroom #3, the hours of operation are Mon - Fri 8:15 AM TO 11:15 AM; AUGUST THROUGH JUNE. Present today were 17 children ages 3 to 5 yrs with 3 Teacher providing care .

During the time of the inspection, LPA observed and reviewed records to verified accuracy:

1. Sampling of children's (5) records (See LIC 857)

2. Sign in and Out sheets were inspected.

3. Emergency contact information observed

4. Parent board observed

5. Staff personnel files reviewed for educational and certification (See LIC 859 )

6. Pediatric CPR and First Aid training for one staff ( Expired 10/30/2022)

7. Mandated Reporter Training Current

8. Emergency Fire Drills were current and up to date

**There is a Parent Board in the classroom that contains Licensing Forms; A sampling of Children's records were reviewed as part of this inspection, Staff are certified and current in Pediatric CPR (exp 10/30/2022)

Center provides snacks and snacks are delivered from the cafeteria. No cooking or dish washing in the preschool activity room; Food is stored appropriately and protected from contamination

Sign in and out sheets were inspected and contain full legal signatures (each child has an individual sign in and out sheet that's kept inside their cubbies

There is a fully stocked first aid kit available and fully charged fire extinguisher. Fire Drill/Earthquake, fire drill is scheduled for Friday , LPA advised lead teacher to conduct a disaster drill every six months.

ADMINISTRATION:
Staff is aware the Department has “full inspection authority” per Health and Safety Code 1596.852, 1596.853, and 1596.535. *There were no excluded individuals present;

Senate Bill AB 633 - Child Care Facilities: Parent Notification Requirements
Summary: This bill amends Health and Safety Code (HSC) sections 1596.859, 1596.8595, 1596.8895, and 1597.05 to improve the transparency of licensing records and to ensure that parents/guardians using a licensed child care facility (Center or family child care home) are aware of situations that present the greatest danger to children. These situations include:
· Serious health and safety violations resulting in Type A citations;

Facility staff are exempt from Community Care Licensing criminal record clearance requirements as criminal record clearances are requirements to be employed by the School District,**LPA observed furniture and equipment to be age appropriate and in good repair. The rooms were found to be clean and safe. Telephone service was verified. Heating, lighting, and ventilation are adequate. Children's belongings are kept along the wall as you enter classrooms. LPA observed age appropriate toys and materials. Flooring and carpet were inspected; Drinking water is available inside the classroom in the form of a water fountain and disposable cups.

· Facility is sharing the boys' and the girls' bathrooms with school students. The bathrooms are located right across from the preschool activity room. During visit, the bathrooms were observed to be safe, sanitary and in operating condition. Boy has 2 toilet, 2 urinals sinks and girls has 4 Toilets, 2 sinks, Number of toilets and sinks in the boys and girls' bathroom could accommodate the licensed capacity.

*All flooring, carpets, and area rugs were observed to be clean. Trash cans/storage containers for solid waste had tight-fitting covers
** The play yard is completely fenced as required; Outdoor play equipment was inspected and found to be age appropriate for this age group of children; Play equipment is in good repair and free of any safety hazards; Center utilizes wood chips under climbing structures. Large play equipment and structures are securely anchored. There are no bodies of water on the campus.

A review of medication policy indicated the center does not provide medication. Incidental Medical Services (IMS) were discussed. Per lead, the facility is currently providing IMS.by school Nurse LPA informed lead Techer to refer to IMS information in the Evaluator Manual – Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. There is a separate area for isolation and care of ill children.

· Non-compliance conferences; or Efforts by the Department to revoke a facility’s license. Each report (documenting a Type A citation) shall remain posted for 30 days along with the Notice of Site Visit (printed out during this inspection). Failure to meet the posting requirements shall result in an immediate civil penalty. In addition, all parents of currently enrolled children and any newly enrolled child for the following 12 months shall receive a copy of report and sign the LIC 9224 acknowledging receipt. Civil Penalty assessments will be assessed if all above requirements are not adhered to.

Lead Teacher is advised to visit www.shotsforschool.org for Immunization information.
Lead Teacher was informed of responsibility to report suspected Child Abuse, 1-800-540-4000.
Lead Teacher is advised for quarterly updates to contact the Child Care Advocates: You can now sign up for Quarterly Updates and PINs for one or more programs through our DSS website at www.ccld.ca.gov. Click on “Receive Important Updates” located in the right middle part of the page, immediately above the Quick links. Put your email address and choose which program(s) you would like to subscribe to and click “subscribe”.

The following deficiencies are being cited in accordance to Title 22 of the California Code of Regulations and/or Health & Safety codes. NONE

Notice of Site Visit has been posted (LIC9213). The notice shall be posted for 30 consecutive days.

Exit interview conducted with Lead Teacher Julie Phillipson. A copy of the Appeal Rights (LIC 9058) were given and explained. Lead Teacher signature on this form acknowledges receipt of these rights.
