25 | On 5/4/23 at 11:40 pm, Licensing Program Analyst (LPA) Patricia Berry conducted a case management investigation. LPA was granted access into the facility and met with Ellie Devitta/Assistant director (AD). LPA toured facility and took a census. There were no children present during the visit.
During the course of an investigation, it was revealed the facility was not following court documents. LPA reviewed the court documents. The court order states, “Access to children’s records: Both the custodial and noncustodial parent have the right to access records and information about their minor children (including medical, dental, and school records) and consult with professionals who are providing services to the children”.
LPA spoke to the Director. Director stated the facility does follow all court documents. Director stated she was willing to provide one of the authorized representatives the current the sign-in/out sheets; however, the authorized representative asked for three months of the sign-in/out sheets and she wasn’t willing to provide
Due to the court order, LPA informed the director the authorized representative has a right to review all documents. Director stated she will follow the court orders for all of the authorized representatives.
Exit interview conducted with Ellie Devitta/AD, report, appeal rights and notice of site visit issued.
Notice of Site Visit must be posted for 30 days.
