13 | On November 16, 2023, Licensing Program Analyst (LPA) Kris Diaz conducted an unannounced
Complaint Investigation Inspection to deliver findings for a complaint received August 29, 2023. LPA was granted access to the center by Lead Infant Teacher, Michella Evans. Per facility representative, the Director is away at an appointment. LPA advised facility representative to contact and inform Director that licensing is present in the facility. LPA conducted a safety inspection where zero deficiencies were noted. At the time of the visit LPA observed 6 infants in care with 2 staff. There were several staff members present during the visit. Director, Alicia Flores arrived approximately 30 minutes after LPAs arrival.
During this investigation, the facility representative provided documentation related to the investigation including children's rosters, staff rosters and schedules, sign-in/out sheets, and vehicle documentation. LPA conducted record reviews, confidential interviews, and observed and recorded facility recorded video.
LPA observation and confidential interviews revealed facility staff handle infants in a rough manner and staff do not follow safe sleep requirements for infants. Cont'd on LIC9099C |
