On March 12, 2025, Licensing Program Analyst (LPA) Isabel Ortega met with Director, Alesha Stenner to conduct an unannounced case management inspection. The purpose of this case management is to follow up on a self reported unusual incident report (UIR) submitted to the Department on 2/14/2025. The unusual incident report is regarding alleged inappropriate behavior between three students.

Upon arrival, there were 27 children in care and 5 staff proving care and supervision. During this inspection LPA was provided with the facility roster, facility profile, and child file for review. LPA toured the classroom where alleged incident took place. In addition, LPA completed a Health and Safety inspection of the facility.

According to interviews conducted, video footage reviewed and observation conducted it has been determined Facility followed protocol and self reported alleged incident within required time frame. According to Licensee supervision is always maintained. During investigation LPA observed teacher #1 and #2 stand by the door while children utilized the restroom after nap time. Facility has a plan to monitor Child #1, Child #2 and Child#3 closely. Parents of children involved were notified by the facility.

Due to interviews conducted and evidence related to the incident, Facility was not cited. No disclosure were made regarding any inappropriate behavior between Child #1, Child #2 and Child #3.

A notice of site visit was provided and requested to be posted for 30 days. An exit interview was conducted, a copy of this report, notice of site visit and appeal rights were provided to facility.
