32 | LPA was provided with two toileting support plans. The initial was dated 09/07/2022 and the subsequent dated 02/03/2023. LPA observed the initial document to disclose the child was only half potty trained, the document did not disclose the child’s specific needs for diapers or pull-up training pants and identified key words the child uses at home when they need to use the bathroom. The subsequent document disclosed the child still needed assistance in verbalizing using the bathroom and would wear underwear during the day and pull-up training pants during nap time.
In addition to toileting support plans, LPA was provided daily logs for the child for the alleged dates the child was picked up in a diaper. LPA observed these logs to document the child’s sleep, nutrition, and toileting events for each day. All daily logs reviewed for the dates supporting this allegation listed diapering/toileting events and a signature by the child’s Authorized Representative. LPA did not observe information regarding the usage of diapers or pull-up training pants on the daily logs.
Statements recorded from staff interviewed revealed children on toileting support plans usually wear pull-up training pants or a combination of underwear. Staff deny the child was placed in a diaper on multiple occasions. Staff corroborated, on one occasion, the facility ran out of pull-up training pants, and the facility utilized diapers in an effort to keep children dry and comfortable. LPA was unable to obtain the date of this occurrence or documents to support this event.
Based on LPA observation, interview and documents collected/reviewed, there was conflicting information received during the investigation from what was alleged, facility staff are not meeting day care child's diapering needs. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
A NOTICE OF SITE VISIT WAS GIVEN. FACILITY REPRESENTATIVE WAS INSTRUCTED TO POSTED IT IN A PROMINENT LOCATION AT THE FACILITY. FACILITY REPRESENATIVE UNDERSTANDS THAT IT MUST REMAIN POSTED FOR THE NEXT 30 DAYS.
An exit interview was conducted, A copy of this report and appeal rights were given to the Facility Representative during this inspection on 04/10/2023. |
