Licensing Program Analyst (LPA) Kim Leung arrived at the facility to conduct an inspection as part of the health and safety review. LPA met with licensee Wei Fen Chen and stated the purpose of the visit. Access to the home was granted. Licensee's primary language is Mandarin and therefore this inspection was conducted in Mandarin. LPA toured the facility, inside and out, records were reviewed and the following was observed and/or discussed:
Normal days and hours of operation are: Monday-Friday, 8am to 6pm
OFF-LIMIT AREAS INCLUDE: Entire upstairs, garage, laundry room, bedroom downstairs and upper slope of back yard.
· The facility is operating within the licensed capacity and appropriate ratios
· The Licensee is present in the home and has ensured that children in care are supervised at this time
· A working telephone is present
· Fully-charged fire extinguisher is present. Smoke detector and carbon monoxide detector are present and were tested by the licensee during this inspection and found to be functioning.
· All hazardous items are inaccessible, this includes: detergents, cleaning compounds, medications and other items which could pose a danger to children
· There is a properly barricaded fire place. Outdoor play areas are fenced (backyard)
· No guns or weapons present as stated by the Licensee. Licensee understands all guns, weapons and ammunition must be key-locked separately and made inaccessible per Title 22 regulations.
· Stairs are barricaded at the bottom at this time
· Home is clean and orderly and ventilation for safety and comfort
· Safe and appropriate toys and equipment are present for both indoor and outdoor activities.
· Verification of control of property on file (owner)
· License, Facility Sketch, Emergency Disaster Plan & Notification of Parent’s Rights poster are posted
· Pediatric CPR and First Aid Card expire on 6/23/2020. Health & Safety Certificate - completed on 3/4/2017.
· There are no bodies of water at this time. Licensee understands all bodies of water including ponds, above

licensure. Existing licensees must meet requirements by March 30, 2018. New employees shall have 90 days to complete training as required. This training requirement may be directly met by using the Department’s Office of Child Abuse Prevention (OCAP) online training modules. The OCAP modules are free of cost and available at: http://www.mandatedreporterca.com/ and are provided in English and Spanish. If no training is made available in a required person’s primary language then those persons shall be exempt from this requirement.

See LIC809D for cited deficiency. Appeal rights discussed and a copy of this report was provided to the licensee on this date. A Civil Penalty has been assessed during this inspection. Payment is due when billed and the check(s) or money orders shall be made payable to the “California Department of Social Services”. YOU WILL RECEIVE AN INVOICE IN THE MAIL. DO NOT SEND MONEY UNTIL YOU RECEIVE YOUR INVOICE. DO NOT SEND CASH.

Licensee has applied for capacity increase to a large family child care. A fire clearance has been obtained on 9/10/2019. The application for capacity increase would be submitted for approval after proof of corrections were received. During the exit interview, licensee Wei Fen Chen confirmed that there are no Registered Sex Offenders living in the facility and/or using the facility address for their mailing address.
As a REMINDER: when your child(ren) turn 18 years of age, you MUST SUBMIT an updated LIC279, LIC508 and TB Screen and have your child submit for LIVESCAN background clearance. This also applies to any adult PRIOR to them moving into the home or who currently lives in the home. Also, PRIOR to employment of any adult, you must submit the LIC508, TB screening and obtain a background clearance through LIVESCAN.

A NOTICE OF SITE VISIT WAS ISSUED AND IS TO BE POSTED IN A PROMINENT LOCATION AT THE FACILITY FOR THE NEXT 30 DAYS. A copy of this report was provided to the licensee on this date and must be made available to the public upon request for the next 3 years.

ground pools & spas, in-ground pools & spas, and some fountains must be properly covered or fenced per Title 22 regulations. The Department must be notified before and after installation of the above types of bodies of water. In addition, all wading pools or similar product must be emptied immediately after use and stored in an upright position.
· Documentation of fire drills on file. The most recent earthquake drill was conducted on 7/8/2019 per drill log.
· Each child’s file contains a copy of the emergency information card with required information. Licensee met AB1207 Mandated Reporter Training and SB792 Immunization Record requirements.
· Criminal record clearances are required prior to all adults living or working in a Family Child Care Home. A civil penalty of $100.00 per day the person has been present, may be assessed. All individuals subject to a criminal record review shall obtain a criminal record clearance or exemption prior to working, residing or volunteering in a licensed home. This was verified on 9/18/2019.
· The Department was granted inspection authority as required by the Health and Safety Code
· Facility is not currently providing IMS at this time. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual – Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. Prior to any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Operation of a Family Child Care Home. Written consent to emergency medical care shall be maintained at the facility for children. This requirement is not met as evidenced by: A review of children's records revealed that valid Consent for Emergency Medical Treatment for 3 of the 4 children in care were not available for review during inspection.

Licensee Wei Fen Chen stated that no additional records were available for review. That presents potential risks to the children's health and safety in the event that emergency medical treatments are needed. That was a repeat violation. Facility was cited for the same deficiency on 9/25/2018.
