*Outdoor area and equipment were inspected for safety, cushioning material (wood chips), good repair and age appropriateness, LPA noted shade, and drinking water from portable dispensers. LPA observed a swing set anchored to the ground, a climbing gym/slide set, bikes and a bike track and park tables. Per Assistant Director there are no bodies of water on the premises. Per Associate Teacher is advised to visit www.shotsforschool.org for Immunization information.
Assistant Director was informed of responsibility to report suspected Child Abuse, 1-800-540-4000.

Facility was observed to be operating according to California Code of Regulations Title 22 and no deficiencies were cited.
Exit interview conducted with Associate Teacher Mary Rodriguez. A copy of the Appeal Rights (LIC 9058) were given and explained. Licensee’s signature on this form acknowledges receipt of these rights.
Notice of Site Visit has been posted (LIC9213). The notice shall be posted for 30 consecutive days. Failure to maintain posting as required will result in a $100.00 civil penalty. Copies of this report must be posted for 30 days in visible location the authorized representatives of children., the children utilize the play center without co-mingling with school-age children.
*Isolation area is in the preschool office along within easy access of a bathroom. mats for this purpose were observed packed away at a secluded place.
