32 | However, it was stated that noises were heard coming from the school age closet, but no rats or mice were seen yet. LPAs also observed stains in some of the carpeted areas throughout the facility and one of the restrooms were observed to be dirty (dark rings inside toilets). In addition, while inspecting playground structures and surrounding areas at the facility, multiple holes were visible around all the structures due to gophers. When weather permits, staff confirmed seeing multiple gophers “popping out” when children are outside playing. Additionally, there were concerns expressed regarding children and/or staff possibly being bitten by the rodents, since children are curious and want to chase the gophers. On 07/03/23, as LPAs were leaving the facility, screams were heard coming from the school age classroom and LPA(s) returned to the facility. Staff then pointed to a mouse that came out of the closet and it was running around the classroom. Children were screaming, as staff were redirecting the children to a different classroom. The mouse was caught during the inspection and thrown away.
This evidence was discussed with the administrator, and it was agreed that facility will be professionally sanitized, and a Pest Control/Rodent exterminator company will be hired to eliminate rodents.
It is licensee's responsibility to continuously reassess playground safety, especially when gophers and/or other rodents have been observed by children and staff ,as well as, to offer visual supervision when children are playing inside and outside.
Based on the information obtained and observations conducted while at the facility. It was confirmed that the facility has a presence of rodents and has not taken appropriate measures to keep the center free of pests. Therefore, the preponderance of evidence standard has been met, the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, divisions & chapter number are being cited on the attached LIC 9099D.)
Exit interview conducted and report was reviewed with Administrator, Mass Amith. Appeal rights were discussed, and A Notice of Site Visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
