On date and time listed, Licensing Program Analysts (LPAs) Kim Leung and E;yse Jones arrived at the facility to conduct an annual inspection. LPAs toured the facility, inside and out, records were reviewed, and the following was observed and/or discussed:
Normal days and hours of operation are: Monday through Friday, 8am to 5pm

OFF-LIMIT AREAS INCLUDE: Laundry room, master bedroom and the bedroom next to the hall bathroom. The laundry room and the bedroom next to the hall bathroom are accessible to children.

The facility is operating within the licensed capacity and appropriate ratios during inspection. However, records revealed that the facility was operating out of ratio this current month on November 1, 8, 9, 10, 16 and 17.


· Appropriate supervision was provided during this inspection

· A working telephone is present and a current number on file

· Appropriate fire extinguisher, smoke detector and carbon monoxide detector present and were tested by the Licensee during this inspection.

· No fireplace

· Hazardous items stored in the kitchen, laundry room and the bedroom next to the hall bathroom are accessible to children

· No weapons as stated by assistant Dalinda Ramos. Licensee understands all guns, weapons and ammunition must be key locked separately and made inaccessible per Title 22 Regulations

· Single Story Home

The garage was off limits as stated at the prelicensing inspection by the licensee. However, LPAs observed children napping the garage during inspection. LPAs observed that the garage is set up as a child care room with tables, chairs, child care furniture and toys there. Assistant Dalinda Ramos stated that children nap and eat in the garage which is a violation of the fire clearance.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. - Facility representative Dalinda Ramos was reminded that all adults residents 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Family Child Care Home. Civil penalties of one hundred dollars ($100) per violation per day for a maximum of five (5) days may be assessed by the Department. Subsequent violations within a twelve (12) month period will result in a civil penalty of one hundred dollars ($100) per violation per day for a maximum of thirty (30) days.
