Licensing Program Analyst (LPA) Ibitoye met with Director, Melissa Davis for the purpose of conducting unannounced Required Annual inspection for school age component. The hours of operation are Monday through Friday from 6:30AM TO 6:00PM. The front of the main building functions as a office area (Room #1) and pre-school area (X367750001).The school age area is located in the rear of the center. There is a Teacher's Lounge at the back of the main building with a staff bathroom.There is a separate detached building that's part of the School-age license, however, due to low enrollment, this building is not in use at this time.Incidental Medical Services (IMS) were discussed.

LPA inspected Kitchen area for safety, cleanliness, proper equipment and protection against contamination and storage. Center serves breakfast, lunch, and afternoon snack (menus posted).

.There are 2 bathrooms designated for school age children, each bathroom has 1 toilet and 1 sink. There's a separate staff bathroom located in the rear of the School-age room. LPA observed the bathrooms to be equipped with soap, toilet paper and paper towels, readily available. Toilets and sinks are functioning properly and age appropriate.

.School age children do not nap during regular school sessions, however, they will have quiet rest time in the summer and during the full day program.

.Outdoor play area was inspected. There is an anchored swing set, teether ball, basketball court and air hockey tables. The area was observed to be free of debris. There is natural shade (trees) and resting areas for the children (benches). Outside Drinking water is provided by a water fountain adjacent to the bungalow (on the backside of the main building). There are no bodies of water observed on the premises. The playground is enclosed by a fence to protect children and to keep them in the outdoor activity area.

.There is a Parent Board (located in the main entrance area) for documents that are required to be posted according to Title 22 Regulations. Sign in and out sheets(facility uses digital Bright wheel) are available. LPA observed fully stocked first aid kits in the office and classrooms; fully charged fire extinguishers; carbon monoxide detectors are incorporated with the centers fire alarm system. Items that can be considered harmful or dangerous to children are located in locked cabinets throughout the facility

Director advised of the requirement to report Unusual Incidents. Licensee informed to utilize the Unusual Incident Report/Injury Report LIC624B when submitting the report to the department (email address on the website: www.unusualincidentreport@dss.ca.gov). A report shall be made to the department by telephone or fax during the department's normal business hours before the close of the next working day following the occurrence during the operation of family day care home. In addition, a written report shall be submitted to the department within seven days following the occurrence of any events specified above.

HEALTH RELATED SERVICES: Medications inaccessible to children; all prescription and non prescription medications have child’s name and are dated; written consent and instruction from child’s representative; a plan to document and report to child’s representative when medication is administered to a child; IMS plan on file (if applicable).

A review of medication policy indicated that prescription medication is administered only with parent's written permission and or doctor prescription. The Director and other administration staff administers medication and documents the dosage, date and time onto a log. Medication is brought and locked at the center (unless parent request daily return). Medication is properly labeled and stored in its original container. Medication is kept at the center until expiration date (given to parent or discarded).

This facility provides Incidental Medical Services – IMS. Advisory/Other: First Aid kit was observed with supplies ( readily available. CPR/First Aid expire 01/13/2026. Mandated Reporter 04/17/2025. Electrical outlets are inaccessible, no baby bouncers saucer chairs, or any recalled and or prohibited toys or sleep/play equipment were observed on the premises. There are no window cords accessible to children.

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Any duly authorized officer, employee, or agent of the Department shall, upon presentation of proper identification, shall inspect the facility. Director shall permit the Department to inspect the family child care home, and to privately interview children or staff, to determine compliance with or to prevent violations of child care center or regulations, also enter and inspect any place providing personal care, supervision and services at any time, with or without advance notice, to secure compliance with, or to prevent a violation.
