On September 30, 2021 at 9:00 AM, Licensing Program Analyst (LPA), Marie Hernandez conducted a case management inspection. LPA met with the Facility Director, Jo Cantwell.

On 09/25/2021, the Director notified the Department that on 09/24/2021, due to major flooding the licensed classrooms #1, #2, #3, & #4 must temporarily relocate for a period of three weeks or longer if needed, to the King Hall and Youth Center on the premises. The four classrooms #1 through #4 are temporarily closed and the children from these rooms are currently not present. The facility is partially temporarily closed until the restoration is completed for those rooms. The projected reopening date is October 20, 2021 or longer if needed.

The new temporary classrooms (King Hall & Youth Center) were previously measured at 1,677 & 3,960 square feet and sufficient to accommodate the children. The facility currently has 95 children enrolled in the program. There are six toilets, two urinals and ten sinks that are exclusive use for the preschool children. The classroom has been set-up for the day with the required supplies. Drinking water is available in both indoor and outdoor play area. There is an approved waiver for the playground for 89 children at one time as per the schedule. All required forms are posted in a prominent area. A fire clearance has been approved for the King Hall & Youth Center on 07/21/2014.

The Director has been advised to notify the Licensing Department when classrooms #1 through #4 have been restored and prior to the children returning.

LPA Marie Hernandez explained the facility evaluation report with the Director. A copy of the report, and the notice of site visit were discussed and provided to the Director. The Director was advised that the notice of site visit must be posted in a prominent place for thirty days. LPA observed the Licensee post the notice of site visit during the visit today.
