25 | On January 20, 2022 at 2:45 PM, Licensing Program Analyst (LPA), Marie Hernandez conducted a case management inspection. LPA met with the Facility Representative, Monique Cortez.
On 09/25/2021, the Director notified the Department that on 09/24/2021, due to major flooding the licensed classrooms #1, #2, #3, & #4 were temporarily relocated to the King Hall and Youth Center on the premises. The four classrooms #1 through #4 were temporarily closed and the children from these classrooms were not present. The facility was partially temporarily closed until the restoration was completed for those classrooms. The Facility Director advised the LPA that the classrooms were restored and ready for an inspection. However, on 12/03/2021, a fire safety inspection was requested due to the major flooding and restoration of the classrooms. On 12/08/2021, the fire marshal approved the fire clearance.
During today's visit, LPA conducted an inspection of the classrooms. The classrooms have been restored and are ready for use by the day care children. Drinking water is available indoors and outdoors in the play area. There is an approved waiver for the playground for 89 children at one time as per the schedule. All required forms are posted in a prominent area. The classrooms are approved for use by the day care children effective today, January 20, 2022.
LPA Marie Hernandez explained the facility evaluation report with the Representative, Monique Cortez. A copy of the report, and the notice of site visit were discussed and provided to the Representative. The Representative was advised that the notice of site visit must be posted in a prominent place for thirty days. LPA observed the Representative post the notice of site visit during the visit today. |
