On 05/18/22 at 12:41 p.m., Licensing Program Analyst (LPA), Rajani Goudreau arrived at the facility to conduct a case management – licensee Initiated inspection. Upon arrival LPA, met with the Director, Veronica Dayag and proceeded to tour the facility. There were 26 napping children in care with two staff members present. The facility is requesting a capacity increase of 100 preschool children, ages 2 years old through kindergarten and a building change under the current licensed address. The daycare building is located near the parish hall in the back-parking lot of the church. Facility operates Monday through Friday from 7:00 a.m. to 5:30 p.m.

Classrooms #1, #2, #3, #4, Art Studio and Stem Lab were inspected and measured a total of 4,581.64 square feet which is sufficient to accommodate 130 children. However, facility is only requesting a capacity of 100 children. Classrooms are equipped with appropriate furniture, napping equipment, including cots, tables, chairs, cubbies, bookshelves, and other materials for the children. The facility has a total of eight toilets and eight sinks exclusive for the children's use, which is sufficient to accommodate the requested capacity. There is a total of three restrooms (one restroom located near the parish building and two restrooms located in the Stem Lab). LPA informed the director children shall be be escorted and supervised by staff members to and from the restrooms and direct visual observation is required at all times. Drinking water is available indoors and outdoors through the use of refillable water pitchers and cups brought from home or provided by the facility. The office and the staff restroom located in the office is designed for ill children, if needed. There is a carbon monoxide detector in the facility and a first aid kit with required supplies. Disinfectants, cleaning solutions, and other hazardous substances are kept inaccessible to the children in care. Required documents are posted in the office. There is an empty water fountain located outside near the office space. Director stated, the fountain will be utilized as a garden for the children. See LIC809-C continuation page…

The outdoor activity spaces were inspected and measured a total of 4,010.20 square feet which is sufficient to accommodate 53 children. There are two outdoor activity spaces available for the children (upper level and lower level). The upper level activity space is behind the office and the lower level activity is near the Stem Lab classroom. Children will access the lower level playground by passing through a parking lot or by going through the stem lab classroom. Plan of operation is needed for the transition of the children to and from the lower level activity space due to the children passing through the church parking lot. Due to insufficient outdoor space a waiver needs to be requested to accommodate the requested capacity. The outdoor play structure on the lower level activity space does not reflect the ages of the children the equipment is suitable for. There is rubber matting underneath the play structure on the lower level activity space. There are play materials on the upper and lower level activity spaces that are age appropriate. The surface of the outdoor activity spaces are maintained, in safe conditions and free of hazards. The outdoor activity spaces are fully fenced. There are building awnings on the upper and lower level activity spaces for a shaded rest area for the children. LPA discussed proper supervision to and from the outdoor activity spaces and placement of staff during outdoor activities.

A fire clearance for 100 preschool children; ages 2 years old through kindergarten was granted on May 11, 2022.



The following is pending with a due date of May 31, 2022:
  • Waiver request with pictures of upper and lower level activity spaces due to insufficient outdoor space.
  • Plan of operation for the transition of children to and from the lower level activity space.
  • Documentation lower level play structure is suitable for ages of children to serve.
  • Pictures reflecting the water fountain inaccessible to the children in care.

No deficiencies issued during today's visit. LPA discussed and provided the following reports: LIC809, LIC809-C, LIC9213-Notice of Site Visit and LIC9058 - Appeal Rights. Once pending documents are received, after final file, and after waiver approval the capacity increase for 100 children; ages 2 years old through kindergarten in classrooms: #1, #2, #3,#4, Stem Lab and Art Studio will be granted. Notice of Site Visit shall be posted for 30 days from today's date.
