25 | Licensing Program Analyst (LPA), Marie Hernandez conducted the case management inspection due to an incident that occurred on 03/02/2020 at 10:20 AM with a fifteen month old child #1. The facility self reported the incident to the Department. The facility reports that child #1 was outside on the playground with a group of other children. There were a total of eight children with the teacher and an aide. Child #1 approached another child to play and that child put his hand out to stop child #1. Child #1 turned and went to walk away, the child clicked his shoes together when attempting to walk and tripped on his own feet. Child #1 fell hitting his brow above the left eye on the post along the bottom of the chain link fence. The teacher stated she observed the incident. The Director attempted to contact the parent but was unable to reach the parent. The Director stated she attempted to contact several authorized representatives but could not reach them either. The Director stated she contacted and spoke to the next authorized representative and advised the representative of the incident. LPA conducted several interviews with the staff and parent. The children could not be interviewed as they are too young, ages 0 months to 24 months. LPA inspected the play yard for health and safety. The infant play yard does not have any high climbing play structures. The play yard has a little tikes tot tree house with a slide for ages six months to five years old. The older infants are only allowed to use the slide. However, the incident requires further review due to insufficient information available at this time.
No deficiency cited today. An exit interview was conducted and a copy of the report and the Notice of Site Visit was provided to the Director. LPA observed the Director post the Notice of Site Visit in a prominent place. The Director states it is understood that this notice must be posted for 30 days. |
