On December 15, 2020 at 11:54 AM, Licensing Program Analyst (LPA), Marie Hernandez, conducted an unannounced Tele-Conference Case Management Inspection due to the COVID -19 State of Emergency to deliver the incident review findings. LPA met with the Facility Director, Lindsey Thornton. The facility self reported the incident to the Department. The facility reports that on 03/02/2020 @ 10:20 AM, a fifteen month old child #1 was outside on the playground with a group of other children when child #1 tripped on his own feet, hitting above his left eyebrow. The facility had to contact 911. Through the course of the incident review, LPA conducted several interviews with the staff and parent. The children could not be interviewed as they are too young, ages 0 months to 24 months. On the day of the incident, there were eight children with the teacher and an aide. Child #1 approached another child to play and that child put his hand out to stop child #1. Child #1 turned and went to walk away, the child clicked his shoes together when attempting to walk and tripped on his own feet. Child #1 fell hitting above his left eyebrow on the post along the bottom of the chain link fence. The teacher stated she observed the incident. The facility contacted 911 and the Director attempted to contact the parent but was unable to reach the parent. The Director stated she contacted and spoke to the next authorized representative and advised the representative of the incident. LPA inspected the play yard for health and safety. The infant play yard does not have any high climbing play structures. The play yard has a little tikes tot tree house with a slide for ages six months to five years old. The older infants are only allowed to use the slide. Per the interviews, the incident was an accident. The child tripped on his own feet, hitting above his left eyebrow.

LPA Marie Hernandez explained the case management incident inspection report, and the Director, Lindsey Thornton stated it was understood. A copy of the report was emailed to the Director. The Director is advised that acknowledgement of the receipt of the report is to be received within 24 hours. NOTE on Facility Signature: SEE FILE FOR ACKNOWLEDGEMENT.
