13 | On December 11, 2020 at 1:04 PM, due to the COVID-19 State of Emergency, Licensing Program Analyst (LPA), Marie Hernandez conducted an unannounced Tele-Conference complaint investigation inspection to deliver the complaint findings with the Facility Representative, Shonna Irving. Present are five children with two staff. Through the course of the complaint investigation, LPA conducted several interviews with the staff, several children including the child in question, and several day care parents. LPA also conducted a virtual inspection of the facility. The staff in question denies placing a blanket over any child’s face including the child in question. The staff in question stated she only placed the blanket on the child in question’s side of his face to block the glare from the sun peering through the window as it was disrupting the child’s nap, but that she did not cover the child’s face or force a child to nap. The staff in question stated she did not place her hand over any child’s nose or mouth if child did not want to nap. The staff in question denied the allegations. |
