Licensing Program Analyst (LPA) Yolanda Baez visited the facility to conduct an annual random inspection. Upon arrival LPA met with Site Supervisor, Claudia Uribe, and proceeded to tour the facility. Director, Lorena Reed, arrived within 20 minutes. The follow ratios were observed:

Room 33 (Half day classroom, serves children 3 - 5 years of age):
  • 1 child was present with 3 staff members
Room 34 (Half day classroom, serves children 3 -5 years of age):
  • this classroom is a part of the head start program and operates Monday through Thursday
  • 5 children were present with 3 staff members
Room 35 (Half day classroom, serves children 3 - 5 years of age):
  • There were not any children or staff members present
Appropriate ratios and capacity were observed.

Furniture and age appropriate equipment is in good condition indoors and outdoors. Children's toilets and hand washing facilities are sanitary. Rooms are safe and clean. Lunches are prepared by the elementary school and brought to the preschool program and storage containers for solid waste are covered. Drinking water is readily accessible inside and outside the classroom. All disinfectants, cleaning solutions, and other hazardous items are inaccessible to children through latches and locks. Storage area for poisons is locked. Solid waste storage vessels, including moveable bins, have tight-fitting covers on, and are in good repair. Outdoor play area is fenced with sufficient material for cushioning. Area has canopies used for shade. There are no bodies of water at this facility. Site Supervisor stated that there are no firearms or ammunition present at the facility. The last emergency drill was conducted on 08/14/2019. There is an operational carbon monoxide detector at the facility. Staff records were reviewed and contain documentation of education, training, and/or experience. Menus are posted. Several children's files were reviewed and are in compliance. First Aid/CPR needs to be reviewed at the office located at 1880 Smythe Ave. Immunization law (SB792) was discussed with Site Supervisor and she understands that anyone who provides care and supervision to the children must have immunization records maintained at the facility for: pertussis, measles, and influenza. An annual continuation will have to be conducted to check center's compliance with SB792 at the Main Preschool Office located 1880 Smythe Ave.

LPA Baez discussed the Mandated Reporter training, AB1207. LPA Baez reminded Site Supervisor that all staff members are to take the training and have the printed certificates present at the facility and available for review. An annual continuation will have to be conducted to check center's compliance with AB1207 at the Main Preschool Office located 1880 Smythe Ave.

LPA reviewed the following with Director: IMS, SIDS, Car seat Law, Shaken Baby Syndrome, the updated CDPH 286 form for "Pre-Kindergarten and School Immunization Record" and Director stated that he understands. This facility does provide Incidental Medical Services- IMS and a written plan of operation has already been submitted to CCL and is on file for the center. There were not any deficiencies issued throughout the areas that were inspected today. NOTICE OF SITE VISIT IS TO BE POSTED FOR 30 DAYS. LPA observed the posting the Notice of Site Visit.

LPA discussed updating the ages that the facility serves because it currently reflects that the facility serves children 2 to 5 years of age but Site Supervisor informed LPA that the center only serves children 3 to 5 years of age. An LIC200A was provided.
