25 | On 2/24/21 at 1:40 PM, Licensing Program Analyst (LPA) Adrian Mangina made an unannounced Case Management-Incident visit to follow-up on two separate self-reported incidents that occurred on 2/17/21 and 2/18/21. Both incidents involved the same child. The first occurred on 2/17/21 when child#1 collided with child#2 and bumped heads and the second incident on 2/18/2021 wherein child#1 fell off a stool hitting head. The inspection was conducted virtually, via Facetime, due to the COVID-19 pandemic restrictions. LPA Mangina met virtually with Director Mindy Coates-Smith and toured the facility. All required notices, forms and licenses were posted.
During the first incident, at approximately 10:00 AM on 2/17/2021, 11 children from the Rosewood classroom (3-4-year-olds) were all playing outside during morning outdoor exploration while supervised by two teachers. Staff #1 stated that she did not witness the accident but was supervising children about 20 feet away, to properly supervise the children. Staff #2 stated that she observed the incident. She said that she was watching five or so children in the garden area outside Atelier Building when she saw Child #1 running towards a friend and accidentally collided bumping heads on impact. Child #2 was uninjured, but Child #1 began to cry and complain of right ear pain. Staff #2 examined the child’s ear and found a red mark there that disappeared shortly after the incident. The child was offered an ice pack but refused. Since the incident, the Rosewood classroom teachers have reminded the children to be more aware of the space they occupy and where their friends are in order to prevent such accidents in the future.
The second incident happened at around 9:00 AM on 2/18/2021 in the Rosewood classroom during morning circle time. There were 9 children being supervised by one teacher. Staff #1 left the classroom and returned just as Child #1 fell. Staff #2 was supervising the children who were celebrating 100 days of school by throwing and catching confetti that the teacher had made. The circle was being held on a large area rug just
